Acute left anterior descending artery occlusion in a hemophiliac A patient during recombinant factor VIII infusion: Treatment with coronary angioplasty

Abstract: Acute myocardial infarction in association with the replacement of recombinant factor VIII in hemophiliacs has not been documented. We describe the use of PTCA in a hemophiliac A patient who developed acute myocardial infarction during factor VIII replacement. Because surgery in hemophiliac A patients remains hazardous, PTCA seems to be an attractive alternative.

“…On the other hand, IHD and thrombosis in persons with hemophilia are thought to be related to factor VIII levels by some investigators [6,7]. Alsolaiman [7] reported coronary artery occlusion during recombinant FVIII infusion in a patient with hemophilia.…”

“…Alsolaiman [7] reported coronary artery occlusion during recombinant FVIII infusion in a patient with hemophilia. Coronary artery disease and myocardial infarction are well-described complications in patients with inhibitors and are presumed to be secondary to the administration of factor concentrates.…”

“…There are few reports of spontaneous arterial (and more specifically coronary) thrombosis [5,6]. There has been a single report of acute coronary arterial occlusion occurring during infusion of recombinant Factor VIII in a patient with haemophilia [7]. Whether the infusion raised the Factor VIII levels to the super-normal levels observed in this case is uncertain, but the thrombosis may have been related to a sudden change in Factor VIII levels.…”
