Acute kidney injury following isotretinoin treatment

Abstract: Patient: Female, 17Final Diagnosis: Acute kidney injurySymptoms: Flank pain • nausea • vomitingMedication: IsotretinoinClinical Procedure: Acne treatmentSpecialty: NephrologyObjective: Unknown etiologyBackground:Isotretinoin is widely used for the treatment of acne that is unresponsive to topical therapy. “…Two case reports have described the possible association between isotretinoin and acute interstitial nephritis. Armaly study group 14 in 2013 reported a case of previously healthy 17-year-old female, with a 5-day history of nausea, vomiting, and bilateral flank pain. Her past medical history only included the use of isotretinoin with a not-defined dose for acne treatment 2 months before admission.…”

“…If patients develop AKI due to rhabdomyolysis, we suggest monitoring BUN, creatinine, and CK levels before initiating/restarting treatment as there have been 5 case reports of isotretinoin-induced AKI in the forms of acute interstitial nephritis, nephrotic syndrome, and hematuria with dysuria. All of these cases endorsed the resolution of AKI with the cessation of isotretinoin and appropriate resuscitation and management [16][17][18]. Although retinoids have been shown to reduce damage and inflammation in experimental models of glomerulonephritis and renal interstitial disease, paradoxically, isotretinoin's xerotic mucosal side effects have been described to cause hematuria [18].…”
