Acute kidney injury due to abdominal compartment syndrome caused by duodenal metastases of prostate cancer

Abstract: Key Clinical MessageOrgan dysfunctions caused by intraabdominal hypertension is named as abdominal compartment syndrome (ACS). A patient with prostate cancer admitted with dyspnea and oliguria. After decompression his health status improved. For patients with malignant disorders presented with oliguria and respiratory problems who have abdominal distension, ACS should be in mind.

“…The aim of decompressive laparotomy in ACS is both to release the IAP and to treat the underlying disease. In our case, and by contrast to the other cancer-associated ACS reported in literature [4, 5, 7, 8, 9, 10], no surgical removal of tumor was done because of the huge tumor size. The specific treatment was based on imatinib tablets taken via the naso-gastric tube during the mechanic ventilation.…”

“…To our knowledge, this is the first case of GIST-associated ACS reported in literature. The other cancer-associated ACS published in the English literature include Wilm's tumor, neuroblastoma, benign and malignant ovarian tumors, prostate and rectal cancers, rhabdomyosarcoma, and Burkitt lymphoma [4, 5, 6, 7, 8, 9, 10].…”

“…A PIA se torna ligeiramente positiva nos pacientes em posição supina, sobretudo quando em ventilação mecânica, devido à transmissão da pressão intratorácica exercida através do diafragma. (14) Segundo Dupont e cols. e Villa e cols., oligúria progredindo para anúria e azotemia pré-renal não responsiva à expansão volêmica caracterizam a disfunção renal da SCA.…”

“…Our PubMed literature survey using keywords of “duodenum or duodenal”, “metastasis or metastatic”, and “cancer or tumor”, did not identify any systematic analyses or reviews highlighting tumor metastases to the duodenum. Several case reports have individually described duodenal metastases from various distant organs, but these metastases mostly arose from the breast ( 1 ), lung ( 2 , 3 ), kidney ( 5 ), prostate ( 7 ), and uterus ( 8 ), and on rare occasions from the liver ( 4 ), colon ( 3 ), bladder ( 6 ), bone ( 9 ), and skin ( 10 ), but never from the ureter. Upper urothelial tract cancers usually metastasize to the lung (4%), liver (12%), and bone (3%) ( 16 ).…”

“…Metastasis to the duodenum has been reported on rare occasions in a limited number of cases of several human malignancies, including those of the breast ( 1 ), lung ( 2 , 3 ), liver ( 4 ), colon ( 3 ), kidney ( 5 ), bladder ( 6 ), prostate ( 7 ), uterus cervix ( 8 ), bone ( 9 ), and skin ( 10 ). However, duodenal metastasis from ureteral cancer has seldom been reported, and its clinical course is unclear.…”
