Acute Dacryocystitis in a 2-Year Old Child Caused by Pantoea

Abstract: A previously healthy 23-month-old girl was admitted for the management of an acute unilateral dacryocystitis following accidental contact with dog faeces. No periocular trauma was reported. Microbiological investigation showed a multiresistant strain of Pantoea species to be the responsible pathogen. The infection responded to a course of oral Clindamycin and Ciprofloxacin, in combination with Chloramphenicol eye drops. This is the first report of an acute dacryocystitis sustained by this microorganism.

“…Septic arthritis or synovitis appears as common clinical outcome of exogenous infection with Pantoea agglomerans [4,5,12,13]. Others include endophthalmitis [14][15][16], acute peculiar bilateral dacryocystitis [17], and periostitis [18]. More frequently, it is found as an opportunistic pathogen in immunocompromised patients, with wound infections, sepsis and urinary tract infections, endocarditis in patients with valvular heart disease, pneumonia post-lung transplant [19], or in patients with chronic renal insufficiency.…”

“…Some Pantoea agglomerans infections associated with hospitalization were defined as sporadic-endogenous-spontaneous infections, as it was not possible to document the source of the germs, and most of the infections were related to a decline in the patient's immune conditions and/or to hospital procedures [17,[31][32][33][34].…”

“…The species of the genus Pantoea can be isolated from plants, water, and soil. Also they have been isolated routinely from Until now, case reports of infections due to P. agglomerans have been reported such as bacteremia in febrile neutropenic patients [4,5], peritonitis in a peritoneal dialysis patient [6], dacryocystitis [7], septic arthritis [8], and wound infections in patients with traumatic injuries [9]. Also an outbreak of P. agglomerans which presented as sepsis in a pediatric urgent care center was formerly reported [10].…”

“…Prompt and effective treatment modalities, based on etiology, are very important to prevent the recurrence and incidence of serious complications as listed above. Congenital nasolacrimal duct obstruction, traumatic obstruction of the nasolacrimal duct, and the presence of a lacrimal foreign body have been reported as major causes of PAD . Application of systemic antibiotics and lacrimal probing are the primary treatments for PAD …”
