Acute bone marrow suppression and gastrointestinal toxicity following acute oral methotrexate overdose

Abstract: Severe toxicity following an acute ingestion of a large amount of methotrexate is rarely reported. The development of toxicity was unexpected in this case given methotrexate's pharmacokinetics and the patient's normal renal function. The serum methotrexate concentrations were below the treatment threshold of the folinic acid rescue therapy nomogram suggesting that the nomogram should not be relied on in acute ingestions. Large acute oral methotrexate poisoning can result in systemic toxicity and folinic acid t… Show more

“…Bidaki et al (9) published a report of two geriatric patients mistakenly given MTX instead of digoxin at the pharmacy, in which one patient died due to development of toxicity. Publications on MTX toxicity are often case reports of oral overdose or mistaken use in place of another drug (10). Cases like ours, where daily parenteral administration of MTX as a presumed analgesic and development of severe neutropenia and neutropenic enterocolitis ending in patient death, are less frequently reported.…”

“…Due to saturable intestinal absorption, MTX absorption is 90% at an oral dose of less than 30 mg/m 2 , whereas it is less than 10-20% when MTX is taken at a dose greater than 80 mg/m 2 . 16 This is another reason why repeated taking of a oral low dose MTX could be more dangerous than an acute single oral overdose, as described in cases of pediatric accidental ingestion and acute overdose [17][18][19] after a reported ingestion of approximately 1000 mg/m 2 . On the other hand, in case of parenteral administration, there is no difference in absorption because MTX is all absorbed.…”

“…The main objective of the SR is to evaluate the efficacy and safety of acupuncture for chemotherapy induced marrow suppression; therefore, the primary outcomes are as follows: white blood cell (WBC) counts, [ 24 ] platelet counts, [ 25 ] hemoglobin counts, [ 26 ] or other validated outcome measures.…”
