Objective assessment of motor function is an important component to evaluating the effectiveness of a rehabilitation process. Such assessments are carried out by clinicians using traditional tests and scales. The Box and Blocks Test (BBT) is one such scale, focusing on manual dexterity evaluation. The score is the maximum number of cubes that a person is able to displace during a time window. In a previous paper, an automated version of the Box and Blocks Test using a Microsoft Kinect sensor was presented, and referred to as the Automated Box and Blocks Test (ABBT). In this paper, the feasibility of ABBT as an automated tool for manual dexterity assessment is discussed. An algorithm, based on image segmentation in CIELab colour space and the Nearest Neighbour (NN) rule, was developed to improve the reliability of automatic cube counting. A pilot study was conducted to assess the hand motor function in people with Parkinson’s disease (PD). Three functional assessments were carried out. The success rate in automatic cube counting was studied by comparing the manual (BBT) and the automatic (ABBT) methods. The additional information provided by the ABBT was analysed to discuss its clinical significance. The results show a high correlation between manual (BBT) and automatic (ABBT) scoring. The lowest average success rate in cube counting for ABBT was 92%. Additionally, the ABBT acquires extra information from the cubes’ displacement, such as the average velocity and the time instants in which the cube was detected. The analysis of this information can be related to indicators of health status (coordination and dexterity). The results showed that the ABBT is a useful tool for automating the assessment of unilateral gross manual dexterity, and provides additional information about the user’s performance.

Parkinson's Disease is a progressive disorder of the nervous system which frequently starts before the sixth decade of life. This is a preliminary overview concerning the effect of a training on cooking with four women with Parkinson's Disease. Cooking is a complex activity that requires functions as attention, memory and task organization; fine motor skills and bimanual coordination are also involved. Moreover, this kind of activity is important for women to maintain their social role as housewives. The assessment was done before and after the training and included: The Unified Parkinson's Disease Rating Scale (UPDRS), Parkinson's Disease Quality of Life questionnaire (PDQL), Schwab & England Scale, Functional Independence Measure (FIM) and a cooking test. The training consisted of 10 group sessions of 90 minutes each one. At the end of the cooking training, the level of disability, measured by the UPDRS, decreased for all participants as well as the time required to perform a cooking test. Scores on other test were unvaried. A cooking training may be useful to improve the course of the Parkinson's disease and should be included in a comprehensive rehabilitation program.

A doença de Parkinson (DP) é caracterizada pelo início assimétrico de sintomas motores e compromete a destreza manual. Espera-se que pacientes com o lado de início da doença preferido comprometido consigam manter bom desempenho com o lado mais afetado (LMA) no teste de destreza manual decorrente da experiência motora ao longo da vida. O objetivo do estudo foi verificar a interferência da coincidência entre o lado de início da doença e preferência manual no desempenho da destreza manual, LMA e lado menos afetado, em pacientes com DP. Os pacientes foram distribuídos conforme o lado de início: Grupo Coincidente (lado preferido acometido) e Grupo Não Coincidente (lado não preferido acometido). A destreza manual foi avaliada pelo teste Annett Pegboard adaptado. A análise estatística ANOVA de dois fatores (grupo x lado), medidas repetidas no último fator, revelou que o Grupo Não Coincidente dispendeu maior tempo para completar o teste com o LMA (p=0,001), enquanto, o Grupo Coincidente dispendeu o mesmo tempo com ambos os lados, indicando que pacientes nos estágios iniciais e com início da doença pelo lado preferido perdem a proficiência manual. Sugere-se que intervenções para melhorar as habilidades manuais sejam aplicadas desde o diagnóstico da doença, principalmente em pacientes com o lado preferido acometido.
