Actinic cheilitis: a systematic review of treatment options

Abstract: Actinic cheilitis is a premalignant condition that can progress to squamous cell carcinoma with a higher propensity for metastasis than cutaneous squamous cell carcinoma. Optimal treatment for actinic cheilitis has not been established, and evidence‐based estimates of clinical cure in the dermatology literature are limited. Here, we review and synthesize outcome data published for patients with actinic cheilitis after treatment with various modalities. A systematic review was conducted in MEDLINE, Embase and t… Show more

“…Treatment options include surgical procedures (vermilionectomy, cryotherapy, laser ablation, and Mohs surgery), conservative modalities (topical treatment using imiquimod, 5-fluorouracil, diclofenac, ingenol mebutate), and photodynamic therapy (3,16,(19)(20)(21).…”

“…However, up to 95% of SCCs on the lip occur on preexisting ACs (2). The metastasis rate is, moreover, four times higher in SCC of the lower lip compared with cutaneous SCC (3). As a consequence, early detection and early and correct treatment are essential for prognosis.…”

“…45 Conversely, certain reports defined AC according to Kaugar's criteria, which requires the presence of epithelial dysplasia. [46][47][48][49] Notwithstanding, five different systematic reviews have proved the superiority of surgical treatments over of non-surgical options in terms of histopathological control, [15][16][17][18]50 particularly partial surgery and laser therapy (CO 2 , Er-Yag, Thulium) alone or in combination with photodynamic therapy elicited the best clinical response, 18 followed by 5% imiquimod-topical chemotherapy drug-(once a day, 3 to 5 days per week for 4 to 6 weeks) and photodynamic therapy. 17,18 However, topical and photodynamic therapy is associated with a higher recurrence.…”
