Acne Fulminans Treated with Isotretinoin and “Pulse” Corticosteroids

Abstract: Acne fulminans (AF) is a rare and severe form of acne that occurs almost exclusively in white adolescent boys. We report AF in a 16-year-old white boy developing 14 months after the onset of uncomplicated acne vulgaris. The symptomatology, etiology, treatment, and prognosis of AF are discussed.

“…10 Systemic isotretinoin has been used successfully in acne fulminans but, paradoxically, may precipitate acne fulminans; accordingly, it is not the initial treatment of choice. 11 So we suggest the simultaneous use of antibiotics and systemic steroids at first. After the acute inflammatory phase of the disease, minocycline was stopped and isotretinoin was started and maintained for preventing recurrence.…”

Systemic isotretinoin has been used successfully in acne fulminans but, paradoxically, may precipitate acne fulminans; accordingly, it is not the initial treatment of choice . So we suggest the simultaneous use of antibiotics and systemic steroids at first.…”

“…The relapse usually occurs 2–8 weeks after the acute attack when the corticosteroids dosage is reduced or stopped; the relapse manifests as worsening of arthralgia and sometimes fever, but usually not as worsening of the cutaneous nodules 2 . Pulsed steroid treatment to control the acute inflammatory phase of the disease may be utilized, with isotretinoin started after the acute inflammatory phase of the disease has been controlled 45 . Systemic corticosteroids (e.g.…”

“…2 Pulsed steroid treatment to control the acute inflammatory phase of the disease may be utilized, with isotretinoin started after the acute inflammatory phase of the disease has been controlled. 45 Systemic corticosteroids (e.g. 0.5-1.0 mg ⁄ kg of prednisolone) are indicated in AF, because they effectively control the skin lesions, reduce fever and also have a favourable effect on the musculoskeletal symptoms.…”
