Acinar Pattern of Mammary Paget?s Disease: A Case Report

Abstract: Mammary Paget's disease (MPD) is an uncommon in situ neoplasm of the nipple-areolar complex and surrounding skin. It is almost always associated with mammary ductal carcinoma in situ (DCIS), high-grade or invasive carcinoma, usually of ductal origin. MPD has only rarely been described in association with specialized forms of breast carcinoma. We report an unusual case of an acinar pattern of MPD with low-grade cytological atypia, associated with invasive papillary carcinoma and micropapillary DCIS of the lacti… Show more

“…An alternative view is that rare cases result from neoplastic transformation of pre‐existing, innocent intraepidermal clear cells of the nipple‐areolar complex (Toker cells) that migrate from nonneoplastic ducts (9,12). Two rare cases of Paget’s disease of nipple with acinar arrangement pointed on architectural resemblance to Toker’s original description of the non‐neoplastic cells that often form glandular structures was presented to support this theory (13,14).…”

“…Это свя-зано с тем, что клиническая картина схожа с другими заболеваниями, такими как атопический или контакт-ный дерматит соска (встречается у беременных и кор-мящих женщин), псориаз (отмечаются также другие локализации данной патологии), эрозивный аденома-тоз соска, гиперкератоз соска и САК, болезнь Боуэна, гистиоцитоз Лангерганса и т. д. [40][41][42]. Эти состояния могут быть подтверждены гистологическими и имму-ногистохимическими исследованиями [28,43].…”

“…10 Immunohistochemistry is a powerful tool in detecting the molecular subtypes of PDB contributing to the staging, therapy selection and prognosis prediction of this disease, meanwhile in differentiated from other nippleassociated lesions including nipple adenoma, malignant melanoma, pagetoid Spitz nevus, florid papillomatosis of the nipple, and so on. [18][19][20][21] In our study we use a immunohistochemical profile including ER, PR, Ki-67, and HER-2 to exam thoroughly the molecular subtypes of PDB which can deep our understanding of this rare disease. Based on classifying different subtypes of PDB, rational therapies will be applicated in clinical cases and a better cure for the disease can be achieved.…”
