SummaryObjectives: This study tested the hypothesis that as pain brings about psychosocial changes in the pain patient, it also does in those relatives dealing with and supporting the patient. In order to test the hypothesis, comparisons of psychosocial status were made among patients with chronic pain, their relatives, and a control group (individuals having no pain and no relative with a pain disorder). Methods: The study included 270 individuals separated into 3 groups: Patients admitted to the Pain Polyclinic at Dokuz Eylül University Faculty of Medicine with chronic pain (Group CP; n=77), relatives of these patients (Group R; n=98), and individuals age-and sex-matched to the relatives as a control group (Group C; n=95). The demographic values of all individuals were recorded, and psychological evaluations performed on all participants using the SCL 90 test. Results: Somatization, anxiety, depression, interindividual sensibility, psychosis, paranoia, anger and general symptom index average scores were found to be high in Group R compared to Group C. Conclusion: Changes in psychosocial status were observed in pain patients and their relatives when compared to the control group. The study recommends that in addition to evaluating the psychological status of pain patients in order to provide them with psychological and medical support, relatives of these patients should also be evaluated carefully and given psychological and medical support to enable necessary diagnosis and treatment.Key words: Patient relatives; chronic pain; psychological evaluation. ÖzetAmaç: Ağrı bireyin kendisinden sonra, ona destek olan yakınında bazı psikososyal değişiklikler meydana getirir. Kronik ağrısı olan hasta yakınları da stresli koşullar altında bulunmaktadır. Bu değişiklikler, hasta yakınının psikolojik durumu, baktıkları hastada kronik ağrıya neden olan hastalığın tipi ve bu hasta bakım yönetimine olan çevre desteği gibi çeşitli faktörlere bağlı olarak da değişmektedir. Çalışmamızın hipotezi ağrının hastalarda olduğu gibi, hasta yakınlarında da psikososyal değişikliklere neden olduğudur. Çalışmamızda bu hipotezin test edilmesi amacıyla kontrol grubu (ağrısı olmayan ve ağrısı olan yakını olmayan kişiler) ile kronik ağrı şikayeti olan hastaların ve bu hastaların yakınlarının psikososyal durumları araştırıldı. Gereç ve Yöntem: Dokuz Eylül Üniversitesi Tıp Fakültesi Algoloji Bilim Dalı Ağrı Polikliniği'ne başvuran ve kronik ağrısı olan ağrı hastaları (Grup KA, n=77) ve bu hastaların yakınları (Grup HY, n=98) ile kronik ağrı hastası yakınına cinsiyet ve yaş olarak eş kontrol grubu olguları (Grup K, n=95) dahil edildi. Context: Quality of life (QoL) and depression are important patient-reported outcomes in cancer care. However, the relative importance of depression severity in predicting QoL remains unclear due to few methodologically sound studies. Objectives:To examine whether depression contributes to impairment of QoL irrespective of prognostic factors and symptom burden.Methods: 563 patients were included from the European Palliative Care Research Collaborative Study (EPCRC-CSA), an international, multi-centre, cross-sectional study. The relative importance of prognostic factors (systemic inflammation (mGPS), co-morbidities and physical performance (KPS), symptom burden (loss of appetite, breathlessness, nausea (ESAS) and pain (BPI)) and depression severity (PHQ-9) in predicting Global Health/QoL (EORTC-QLQ-C30) were assessed using hierarchical multiple regression models.Results: 55% were females, median age 64 years, 87% had metastatic disease, Conclusion:Depression severity was the strongest single predictor of poorer QoL in this sample of patients with advanced cancer, after accounting for a wide range of clinically relevant variables. Future studies should investigate the contribution of psychosocial variables on QoL. Our findings emphasize the importance of managing depression to achieve the best possible QoL for these patients.

Abstract: The HADS anxiety subscale performed worse than the total and the depression subscales for both indicators. Diagnostic accuracy varied widely by threshold but was consistently superior for depression screening than for screening of any mental disorder.
