Background: Accurate estimation of haemoglobin (Hb) is an essential prerequisite for detection of anaemia and for assessing response to treatment. In India the gold standard cyanmethaemoglobin method is used for estimation of Hb in tertiary care institutions but accurate methods for Hb estimation are not being used in primary and secondary healthcare settings. Major national surveys have demonstrated the feasibility and accuracy of Hb estimation by indirect cyanmethaemoglobin method (ICM). However, ICM has not been used for Hb estimation in point of care because of reports that ICM underestimates Hb. A study was taken up to compare the accuracy of Hb estimation by direct (DCM) and indirect cyanmethaemoglobin (ICM) method.Methods: ANMs and laboratory technicians pipetted out duplicate samples of blood for both direct and indirect Hb estimation. Another trained laboratory technician undertook Hb estimation using colorimeter in all samples. Hb values between duplicates were compared.Results: There was excellent concordance between means and frequency distribution of Hb values between duplicates when Hb in both were estimated using DCM or ICM and also when Hb estimation was done by DCM in one and ICM in the other. Bland Altman Plots confirmed that across Hb levels there was excellent concordance between direct and indirect method of Hb estimation.Conclusions: Indirect cyanmethaemoglobin method is accurate. It can be used at point of care for screening for anaemia and assessing improvement in Hb after treatment especially in primary and secondary health care settings.

De acuerdo con la Organización Mundial de la Salud (OMS), hay 802 millones de personas entre niños en edad pre-escolar, y mujeres en edad reproductiva que son diagnosticadas de anemia. Estas cifras muestran un panorama poco alentador para los gobiernos, puesto que vemos el gran énfasis en los programas de suplementación de hierro, con distribución a nivel nacional. Sin embargo, los valores reportados en las encuestas y las grandes bases de datos que se utilizan para la elaboración de estadísticas nacionales se basa en la medición de la hemoglobina mediante el equipo HemoCue®, que presenta variantes entre los equipos y que reportan valores que no permiten determinar la causa de la anemia, basando la intervención solo en la suplementación de hierro, cuando a nivel mundial se ha determinado que solo el 50% de casos de anemia se deben a deficiencia de hierro, 42% a causas inflamatorias y 8% a otras causas. En el reporte de 2016, la OMS recomendó como primera prioridad la evaluación del hemograma completo para definir anemia, y cuando no exista esta facilidad usar la medición de la hemoglobina. En la presente revisión narrativa se hará un análisis de cada metodología y la utilidad tanto para la práctica clínica como para el uso poblacional en el diagnóstico de la anemia gestacional.

Potential new tool for anemia screening: An evaluation of the performance and usability of the TrueHb Hemometer

In low-and middle-income countries, many women experience anemia during pregnancy due to insufficient dietary intake of key micronutrients, parasitic infections, hemoglobinopathies, and chronic infections. Maternal anemia increases perinatal risks for both mothers and infants, and slow progress to reduce the prevalence may be due in part to the lack of affordable tools to quantify hemoglobin levels in antenatal care (ANC) clinics. A simple, inexpensive, accurate, and robust diagnostic is needed to measure hemoglobin in ANC. This study evaluated the performance and usability of the TrueHb Hemometer. A crosssectional diagnostic accuracy study was conducted to compare the accuracy of the TrueHb and the HemoCue® 201+ using capillary samples. Next, analytical performance (precision, coefficient of variation, R 2) of the TrueHb was evaluated in varying environmental conditions using linearity panels with serial dilutions of venous blood samples. Lastly, the usability of the TrueHb Hemometer was assessed across three domains (effectiveness, efficiency, and satisfaction) by 20 ANC providers in Ghana. Capillary blood test results were not well correlated (R 2 = 0.35) between the TrueHB and HemoCue201+, but 80% of TrueHb measurements were within +/-1.0 g/dl of the HemoCue® 201+ hemoglobin values. Precision tests indicated similar mean values across the three environmental conditions (CV<6%). At 21˚C, the TrueHb follows a linear relationship (R 2 �0.96) but does not generate accurate readings below 4.0 g/dl. At 30˚C and 37˚C, the TrueHb follows a linear relationship (R 2 > 0.90) but begins to underestimate the hemoglobin concentration below 7.0 g/dl. The usability study identified potential failure modes due to inadequate instructions and device feedback. With some modifications, both to the product and to the instructions for use, the TrueHb may be suitable for use in ANC settings to help fill the diagnostic gap for anemia screening during pregnancy. Further testing is required with anemic populations in LMIC settings.
