Purpose: To evaluate the accuracy and limits of a frozen section analysis in our department in order to develop a quality control. Materials and methods: all frozen sections reported at our department between 1st January 2010 and 31st December 2010, have been retrospectively evaluated. The frozen section results were compared to permanent paraffin section results. Results: Frozen sections and final diagnosis agreed in 95% and disagreed in 5% (13 cases), 6.6% of the cases were deferred (17 cases). The most common pathological processes encountered were presence/typing of neoplasm (85.4%) and assessment of surgical margins (7.3%). Conclusion: The accuracy of frozen sections diagnosis at our department can be interpreted as comparable with the most international quality control series and is valuable to assist surgeons in their decision

Background: Diagnostic accuracy of intra-operative frozen section (FS) depends largely on quality of tissue sections backed by good clinical communication and experience of reporting pathologist. Periodic audit of this consultation in surgical pathology help in assessing the efficiency of procedure and addressing the pitfalls. In this study authors have analysed the spectrum, indications and assessed the accuracy of FS consultation in their institution.Methods: A retrospective study of 212 consequent tissue specimens submitted for FS over two years in study centre was conducted. The FS and corresponding formalin fixed paraffin embedded (FFPE) tissue section with their final histopathological examination (HPE) reports were studied and analyzed. The results were classified in concordant, discordant and deferred categories. Accuracy rates and discordant frequencies were calculated and comparison with other similar studies was done. Reasons for inaccuracies were deduced.Results: A total of 212 tissue specimens for FS were reported over two years in this study institute, six of which showed discordant results. Most common site of FS in this study centre was from central nervous system (CNS) lesions (28.77%). Indications for intra-operative consultation were mainly for establishment of tumor diagnosis (66.51%) and status of margins (29.25%). The accuracy rate was found to be 97.17% with error rate of 2.83%. On analysis of discordant cases; the reason for inaccuracy was mainly due to interpretation error (83.33%).Conclusions: The audit of FS consultation established that accuracy rates of this study institution are comparable with most international quality control statistics for FS. The discordant cases were mostly false positive hence emphasising that a variable degree of reservation is required while interpreting and communicating the FS results. The closest possible diagnosis should be communicated on FS and definitive diagnosis should be deferred to HPE in case of doubt.

BACKGROUND There are two main types of tumours-malignant or cancerous tumours and benign tumours. Cancerous tumours can be divided into primary tumours that start within the brain and secondary tumours that have spread from somewhere else known as brain metastasis tumours. Primary CNS tumours represent 2% of estimated new cancers occurring in adults. The incidence of all adult primary brain tumours in the United States is 11.8 per 1,00,000 persons per year. Frozen Section (FS) diagnosis is a useful diagnostic technique for diagnosis of tumours of central nervous system as it guides a surgeon with appropriate therapeutic decision-making on most occasions. MATERIALS AND METHODS In this retrospective study, we reviewed the reports of all CNS lesions from July 2016 to August 2017 in the Department of Pathology Jawaharlal Nehru Medical College, Sawangi (M), Wardha. Specimens were received from the OT complex of Neurosurgery Department of Acharya Vinoba Bhave Rural Hospital, Sawangi (M), Wardha. A total of 30 cases were studied. The tissue was frozen in a cryostat (-25°C) and 6-7μ sections were prepared and stained with rapid Haematoxylin and Eosin (H and E). The slides were examined under a light microscope by one or more general pathologists. The same specimen along with some more resected specimen were formalin fixed, additional representative sections were taken for paraffin embedding and routine H and E staining. RESULTS When frozen section diagnosis was correlated with histopathological diagnosis, it was found that amongst 30 cases, 21 frozen section diagnosis correlated with the standard histopathological diagnosis, which accounted for 70% diagnosis of the total cases. The sensitivity was calculated as 70%. CONCLUSION The final diagnoses were based on permanent sections and IHC studies. On correlation, the 'p' value was found to be > than 0.005, which is significant. These findings are concordant with the findings of our study.

Abstract: Accuracy of our intraoperative consultation is comparable with other centers in Western countries. Most of the discrepancies can be prevented by providing more clinical information for the pathologist and more accurate sampling.
