Accuracy of a combined insulin-like growth factor-binding protein-1/interleukin-6 test (Premaquick) in predicting delivery in women with threatened preterm labor

Abstract: This novel triple biomarker model of native and total IGFBP-1 and IL-6 appears to be an accurate test in predicting spontaneous deliveries and spontaneous exclusive preterm deliveries in threatened preterm labor in singleton pregnancies.

“…Premaquick performed well among women with threatened preterm labor, predicting delivery within 14 days of enrollment with high sensitivity, specificity, PPV, NPV, and accuracy. This finding corroborates a previous Nigerian study on Premaquick by Eleje et al., and is consistent with a study in Kuwait by Abu‐Faza et al., who noted high specificity and PPV with Premaquick but relatively low sensitivity (39.8%), NPV (62.2%), and accuracy (68.2%). The low sensitivity, NPV, and accuracy observed by Abu‐Faza et al .…”

“…Second, among the women who did not deliver within 14 days of enrollment, some were not followed until delivery. Thus, a 14‐day cut‐off was selected on the basis of the protocol reported in previous studies on preterm labor prediction . Third, the study did not determine the cost‐effectiveness of the two tests, which remains a topic for further research.…”

“…Although previous study on this combination of biomarkers has revealed that they are useful in predicting preterm labor . However, term and preterm labor events share common pathways of cervical ripening, even though the instigating prompts may differ .…”

“…IGFBP-1 has the advantage of not being influenced by recent sexual intercourse or urine contaminants [3][4][5][6]. Eleje and coworkers [7] found that the determination of vaginal fluid IGFBP-1 (native and total) and IL-6 is a valuable method for the prediction of spontaneous delivery and preterm delivery in women at risk of preterm labor and, at present, has the best negative and positive predictive values for preterm birth. If this finding is supported by other groups and studies, it could help tremendously in the clinical practice of obstetrics.…”
