Accessing rehabilitation after stroke – a guessing game?

Abstract: Aim: To explore the use, meaning and value of the term ''rehabilitation potential''. Method: The authors of this commentary met to discuss concerns relating to the pressure on health service staff created by reduced length of stay in acute settings of those who have suffered a stroke and the need to determine the potential of a patient for rehabilitation in order to inform discharge arrangements. Points raised at this meeting were shared with an email group who over a 12-month period contributed to this paper.… Show more

“…Multimodal interventions might be favorable to introduce in various settings, and be incorporated into different rehabilitation pathways, as the stroke survivors needs and willingness to participate are likely to change over time [ 42 ]. The positive experiences of multimodal stimulation may be initiated already during hospital stay, where an enriched protocol was found to be beneficial early in the rehabilitation process [ 12 , 13 ].…”

“…Acute therapies for ischemic stroke are based on the re-canalization of occluded vessels through pharmacologic and invasive surgical procedures. In contrast, during the chronic stage, physical and cognitive rehabilitation therapies might work in a minority of patients, especially in subjects with less extensive damage after the initial insult [2,3]. It is clinically accepted that the administration of a tissue plasminogen activator (t-PA) for clot dissolution-alone or in combination with surgical procedures such as endovascular thrombectomy for clot retrieval-constitute the most acceptable treatment to treat stroke patients in the early stages (acute phase).…”
