Abnormal Placentation

Abstract: Abnormal placentation poses a diagnostic and treatment challenge for all providers caring for pregnant women. As one of the leading causes of postpartum hemorrhage, abnormal placentation involves the attachment of placental villi directly to the myometrium with potentially deeper invasion into the uterine wall or surrounding organs. Surgical procedures that disrupt the integrity of uterus, including cesarean section, dilatation and curettage, and myomectomy, have been implicated as key risk factors for placent… Show more

“…Despite its cost and unavailability in many centers, MRI should be reserved for cases with inconclusive sonographic findings (13,15,17). Recently, 3D ultrasound and the 3D power Doppler have been introduced for the detection of MAP, and MSV technology was introduced in 2004 (28). MSV represents the sequential sections of the scanned ROI, similar to MRI technology.…”

“…Abnormal placentation (accreta, increta and percreta) has emerged over uterine atony as the leading indication for peripartum hysterectomy. However, these placental abnormalities rarely get detected before delivery [1][2][3]. Once a rare occurrence, morbidly adherent placenta is now becoming an increasingly common complication of pregnancy, mainly due to the increasing rate of Caesarean delivery over the past 50 years [4].…”

“…Placenta creta, often associated with massive postpartum hemorrhage and carrying significant maternal morbidity, is one of the leading indications for gravid hysterectomies (2). In a study by Forna et al, only 12 cases were diagnosed from 1990 to 2002 in their series of 70,449 births (10).…”

“…Additionally, nearly all invasive procedures that were thought to be minor in nature, such as uterine curettage, hysteroscopic surgery, myomectomy, endometrial ablation, and uterine artery embolization, have been associated with a higher incidence of placenta creta (2). The relative contributions of these risk factors remain unknown (1).…”
