Introduction: This article discusses the path of healthcare associated infections (HAI) indicators in the intensive care unit (ICU) of a public teaching hospital in Belo Horizonte, Minas Gerais, Brazil, after certain change in its nursing staff: pair of nursing caregivers. The model of a pair of caregivers consists in assigning one nurse and one nursing technician for every three patients. The indicators analyzed were infection related to central venous catheters (CVCs), the risk of HAI, turnover, and absenteeism. Objective: The objective of this paper is to understand the impact of the restructuring of the nursing staff in Human Resources and on the rate of infection in the ICU. Methods: As for methods, it is a qualitative and descriptive research carried out as a case study. Results: The results have shown that the risk of HAIs significantly increased after the change in staffing, but the density of vascular access infection associated with CVCs was drastically reduced. The results of turnover of nursing technics decreased and the turnover of nurses increased while the absenteeism of the nursing team decreased after the change. The interviews revealed that there was a gain at the care due to the change. Conclusion: As a conclusion, the results of the study have shown that the proposed nursing model caused a care gain, once the interviews exposed that and indicator directly related to nursing team care (infection associated with CVCs) decreased.

BACKGROUND AND OBJECTIVES: Pain and stress are the main factors stimulating behavioral, physiological, and hormonal changes in preterm newborns (PTNB) admitted to a Neonatal Intensive Care Unit (NICU); therefore, the systematic multidimensional assessment of pain at the bedside becomes an essential tool for reducing the impact of brain stimulation, as it guides the appropriate treatment. The objectives of this study were to assess the pain at the bedside of PTNB submitted to peripheral venipuncture or diaper change and to describe the measures for pain relief. METHODS: Longitudinal study, with a prospective cohort of 25 PTNB, carried out in an NICU in the city of Rio de Janeiro. Pain assessment at the bedside was performed at three different times, using the Neonatal Infant Pain Scale (NIPS) and Premature Infant Pain Profile (PIPP), when subjected to peripheral venipuncture for infusion therapy and diaper changes. The scores of the scales were analyzed using the Chi-square test. RESULTS: In the puncture group, there was pain and increased intensity. In the diaper group, three PTNB showed pain at the third assessment. During peripheral venipuncture, the non--pharmacological measures were used: oral 25% glucose, non--nutritive suction and a combination of the two. CONCLUSION: Assessing pain at the bedside using scales helps nursing professionals in pain prevention and treatment, avoiding altered responses of newborns in the face of procedures. Objetivou-se compreender o cuidado do enfermeiro com a inserção do Cateter Central de Inserção Periférica em recém-nascidos prematuros, de modo a identificar se a prática segue garante segurança ao paciente. Trata-se de uma pesquisa de campo, descritiva, com abordagem qualitativa, realizada em hospital de assistência terciária, localizado no sudoeste do estado do Paraná, com nove enfermeiros das Unidade de Terapia Intensiva Neonatal e Unidade de Cuidados Intermediários Neonatal. A coleta de dados aconteceu mediante aplicação de checklist e uma entrevista semiestruturada gravada, com auxílio de um gravador, cujas respostas foram transcritas e agrupadas por semelhanças. Classificaram-nas em duas categorias: Práticas que promovem segurança ao neonato durante a utilização do PICC; e Práticas que promovem conforto ao neonato durante a inserção do PICC. Compreende-se a importância de conhecer as medidas de segurança e conforto, além de desenvolver ações adequadas, para que se possa garanti-las. Premature patient sefety in the introduction and maintenance of the peripheral insertion central catheter Abstract: The objective was to understand the care provided by nurses with the insertion of the Peripherally Inserted Central Catheter in preterm newborns, in order to identify whether the practice continues to guarantee patient safety. This is a field research, descriptive, with a qualitative approach, carried out in a tertiary care hospital, located in the southwest of the state of Paraná, with nine nurses from the Neonatal Intensive Care Unit and the Neonatal Intermediate Care Unit. Data collection took place by applying a checklist and a semi-structured recorded interview, with the aid of a recorder, whose answers were transcribed and grouped by similarities. They classified them into two categories: Practices that promote safety to the newborn while using the PICC; and Practices that promote comfort to the newborn during the insertion of the PICC. The importance of knowing the safety and comfort measures is understood, in addition to developing appropriate actions, so that they can be guaranteed. Se trata de una investigación de campo, descriptiva, con abordaje cualitativo, realizada en un hospital de tercer nivel, ubicado en el suroeste del estado de Paraná, con nueve enfermeros de la Unidad de Cuidados Intensivos Neonatales y la Unidad de Cuidados Intermedios Neonatales. La recolección de datos se realizó mediante la aplicación de un checklist y una entrevista grabada semiestructurada, con la ayuda de una grabadora, cuyas respuestas fueron transcritas y agrupadas por similitudes. Los clasificaron en dos categorías: Prácticas que promueven la seguridad del recién nacido durante el uso del PICC; y Prácticas que promueven la comodidad del recién nacido durante la inserción del PICC. Se entiende la importancia de conocer las medidas de seguridad y confort, además de desarrollar acciones adecuadas, para que puedan garantizarse.
