Our aim in this study is to retrospectively analyze the results of Targeted Temperature Management (TTM) treatment in our intensive care unit. Following the ethics committee's approval, the demographic data, cardiac arrest history, the follow-up process, and the final status of patients who were applied TTM in Sakarya University Training and Research Hospital Anesthesia Intensive Care Unit between January 2017 and December 2019 were recorded. Then, the effect of these parameters on mortality was evaluated using regression analysis. Data of 47 patients who met the study criteria were analyzed. The mean age of the patients was 50 ± 16 and 14 (29.8%) patients were female. The duration of cardiopulmonary resuscitation (CPR) was 22.35 ± 16.08 minutes, 35 (74.5%) patients had a witness to arrest, 25 (53.2%) patients were arrested in the hospital, and 13 (27.7%) patients had shockable rhythm. It was determined that 60% of the patients died, 20% of them were care patients with poor neurological outcomes and 20% of them were discharged outpatient with good neurological outcomes. Age, development of acute renal failure, requiring vasopressor, high APACHE scores, and length of hospital stay were found to be independent risk factors for mortality.

We present the case of a 59-year-old gentleman with a history of nonmetastatic tonsillar malignancy and radiation chondronecrosis, who underwent targeted temperature management (TTM) in a sequential manner. The first time the patient underwent therapeutic cooling occurred after he developed a respiratory arrest followed by a cardiac arrest and prolonged hypoxemia after a diagnostic laryngoscopy. The patient was kept at 32°C for 24 hours, and 48 hours after rewarming woke up neurologically intact. However, six hours post-extubation, he suffered upper airway obstruction, followed by a prolonged cardiac arrest. Return of spontaneous circulation on this second episode was achieved after 45 minutes of resuscitation maneuvers. The patient was cooled again and kept at 32°C for 48 hours. Five days later, the patient recovered, with an intact neurologically function. This case stands out the importance of sequential TTM after repeat cardiac arrests with a return of spontaneous circulation (ROSC), demonstrating this process as a neuroprotective way of treatment.

Introduction. The aim of the experiment was to find the relationship between the extent of myocardial infarction and the internal body temperature of the rats. Our second goal was to investigate if mild hypothermia applied during myocardial ischemia and reperfusion brings cardioprotective effects.Materials and methods. The study was performed in vivo on rat hearts. Myocardial infarction was induced by 30 minutes of the left anterior descending artery occlusion followed by 60 min reperfusion. Thirteen rats were assigned to a group depending on the internal body temperature (t) ≤ 35.5°C (n = 5), 35.6 ≥ t ≥ 37.5°C (n = 4) and t ≥ 37.6°C (n = 4). The desired temperature was achieved during pre-ischemic procedures. The presence of the infarction scar and its size were assessed in planimetry. Infraction size was calculated as the infarct area relative to the area at risk (IA/RA).Results. The incidence of infarction scar in the groups of rats with body temperature ≥ 35.6°C was significantly higher (p < 0.01) compared to the group with the temperature ≤ 35.5°C.There was statistically significant positive correlation (r = 0.787, p < 0.01) between IA/RA and the internal body temperature of the rats. Conclusions.Any reduction in the body temperature during myocardial ischemia and reperfusion seems to be beneficial for the rat's myocardium. Moreover, hypothermia ≤ 35.5ºC applied during ischemia and reperfusion fully prevents from myocardial infarction. We believe that there is a great need to intensify research on therapeutic hypothermia in humans, so that this method of treatment could be finally used in the course of myocardial infarction.
