Although real-time three-dimensional echocardiography (RT-3DE) can display stereoscopic images, its acquisition and image display quality are affected by many factors. So, the study aimed to evaluate the importance of factors affecting the efficiency of RT-3DE image display quality, and establish an optimized method for RT-3DE examination and image processing in children. Based on the specific cardiac acoustic characteristics in children, 83 healthy children were studied and divided into five groups. Three aspects of seven factors were selected for RT-3DE examination and analysis of orthogonal test design method, including acquisition windows, gain, compress, post process, smoothing, frequency fusion and 3D Vision. The principal effective analysis of orthogonal test design method and its table L(18)3(7) were used. The efficiency rate of three-dimensional image display quality was used as the inspection index for orthogonal test design method. From our findings, according to the average deviation orders of seven factors, the maximal average deviation was the factor of gain. The aspects with maximal average deviation of each factor were at via-subcostal and via-apical acoustic windows, 90 of gain, 60 of compress, E of post process, five of smoothing, F3 of frequency fusion and A to B of 3D Vision, respectively. Namely, the optimized method for RT-3DE examination and image displaying could be achieved by setting the factors. Multi-factorial analytical study on RT-3DE cardiac image display quality could be helpful to promote clinical application of RT-3DE, and be conducive to design a multi-parameter presetting of ultrasound system to optimize RT-3DE examination.

Augmented reality (AR) applications link real with virtual image data, in order to increase their information content. In medicine they are especially useful for education and for supporting the interpretation of three-dimensional (3D) image data. Simulators are used to train risky or expensive procedures. In the AR application EchoCom2 a 3D surface model of the human heart is linked with echocardiographic volume data sets. The 3D echocardiographic data sets are registered with the heart model to synchronize it's temporal and spatial orientation. The heart model together with an animated ultrasound sector represents a reference scenario, which displays the currently selected cutting plane within the echocardiographic volume data set. Modifications of the cutting plane within the echocardiographic data are transferred simultaneously and in real time to the reference scenario. The AR application is used as a simulator to train two-dimensional echocardiographic examinations and as an orientation and navigation aid for the exploration of 3D echocardiographic data sets. Beginners in echocardiography have only a rudimentary conception of the spatial relationship between the actual ultrasound image and the 3D anatomy of the heart. They are unable to translate multiple two-dimensional slices into a coherent 3D mental image of the heart. In EchoCom2 the trainee can interactively explore the 3D heart model and the registered 3D echocardiographic data sets by the animated ultrasound sector, whose position is controlled by an electromagnetic orientation and position system (EPOS). The data from the EPOS are used to calculate the echocardiographic images that are analogue to the position of the animated ultrasound sector. EchoCom2 is also used to support the interpretation of 3D echocardiographic data sets. The analysis of 3D echocardiographic data has to be done during a post processing. Defining the exact position of a cutting plane within the volume is difficult due to the lack of a standardized representation, the independence of the cutting plane of any transducer position and the possibility to calculate an indefinite number of views. The simultaneous representation of the current cutting plane both in the volume data, and in the heart model enables the examiner ad hoc to recognize it's position and the visualized structures.

The purpose of this study was to compare gated with nongated three‐dimensional fetal echocardiography in terms of the ability to demonstrate fetal cardiac anatomy. We examined nine fetuses in utero using conventional two‐dimensional sonographic imaging equipment, an electromagnetic position sensor, and a computer‐graphics workstation. Free‐hand sweeps were performed through the fetal heart and great vessels in either transverse or sagittal orientations with respect to the fetal heart. Seven transverse and five sagittal sweeps were selected for reconstruction and analysis. Cardiac gating was performed by using a temporal Fourier transform to determine the fundamental frequency of cardiac motion. Two‐dimensional data from each sweep were reprojected to a series of volume data sets. Each series was then condensed to a single volume, so that each two‐dimensional sweep could be compared with its respective gated and nongated volume data sets. The two‐dimensional data were reviewed utilizing a display with forward and backward cineloop capability. The gated and nongated volume data sets were displayed interactively as a series of three orthogonal planes, with the ability of the observer to control the location of each image plane within the volume. The gated data were animated with variable display frame rates. Conventional two‐dimensional imaging provided a fairly complete evaluation of the fetal heart when scanning included the four‐chamber view with a sweep across the outflow tracts. Nongated three‐dimensional fetal echocardiography allowed visualization of some structures and views not demonstrated with two‐dimensional ultrasonography. Gated three‐dimensional fetal echocardiography provided significantly better visualization and comprehension of cardiac anatomy than nongated three‐dimensional fetal echocardiography. The superiority of gated over nongated three‐dimensional fetal echocardiography appears to come from both improved image quality and the anatomic clues that derive from the ability to view cardiac motion.
