A successfully treated case of mycetoma due to Nocardia veterana

Abstract: We report a successfully treated case of mycetoma from which an unusual Nocardia species was isolated. The isolate was identified as N. veterana by biochemical characterization and 16S ribosomal RNA gene sequencing, and it has not been previously reported as a causative agent of human mycetomas. Treatment with various antibiotics over 6 years and surgical resection failed to cure the disease. However, the combination of intravenous imipenem/cilastatin and amikacin along with oral clarithromycin and minocycline… Show more

“…24 In addition, case reports have described the efficacy of imipenem and amikacin combination therapy for S somaliensis and N veterana actinomycetomas. 25,26 Our study demonstrates the efficacy and tolerability of imipenem, both as monotherapy and in combination with amikacin, in the management of severe and refractory actinomycetomas. Although 50% of patients were not cured, they still had significant clinical improvement and negative culture result, which hitherto they had not achieved with previous antibiotic therapies.…”

“…The mainstay of antimicrobial therapy of nocardial infection has been sulfonamide combination. Other medications that have been used with efficacy against certain actinomycetes include dapsone, amikacin, and imipenem 31,32 . A combination therapy of amikacin, cotrimoxazole, and rifampicin has been proven effective for treatment of resistant cases of actinomycetoma 33…”
