Introduction: Stroke is a sudden-onset neurologic deficit and having a type 2 diabetes increases the risk of stroke twice as much. In addition to affecting physical and mental abilities, stroke can also lead to a decline in nutritional status. Malnutrition is associated with poor functional outcomes in all stroke subtypes. The Controlling Nutritional Status (CONUT) score is an objective nutritional marker and could reflects malnutrition due to its comprehensive assessment of nutritional status. This study aims to find the association between nutritional status towards the outcome of acute ischemic stroke patients with type 2 diabetes. Method: This was an observational analytic study with prospective cohort design using primary data collected consecutively from patients with both acute ischemic stroke and type 2 diabetes mellitus who were hospitalized in the Stroke Corner Room, Intensive Care Unit (ICU) and inpatient ward of Haji Adam Malik General Hospital who met the inclusion criteria. The nutritional statuses of patients were measured using the CONUT score and patients outcomes were measured by mRS score. Spearman correlation test was used to assess the association between nutritional status and outcome of acute ischemic stroke patients with type 2 diabetes mellitus. Results: There were 43 patients who met the inclusion criteria where the majority of patients were male 22 people (51.2%), in the age group 51-65 years 23 people (53.5%) with a mean age of 57.93 + 10.01, was a housewife 15 people (34.9%) and Bataknese 27 people (62.8%). The majority of patients in this study had mild malnutrition with a total of 15 people (34.9%) and a median CONUT score of 3 (0-11) and poor outcome as many as 29 people (67.4%) with mRS score median 4 (0-6). The result of Spearman correlation test showed a significant correlation between nutritional status and the outcome of acute ischemic stroke patients with type 2 diabetes mellitus with a moderate correlation. Conclusion: There was a significant correlation between nutritional status and the outcome of acute ischemic stroke patients with type 2 diabetes mellitus with a moderate correlation

Objective: To find out the frequency of hypoalbuminemia and mortality rate in cases of acute ischemic stroke. Material and methods: Between April 2020 to October 2020, total 180 patients of acute ischemic stroke, age between 40-60 years of both gender were selected from Department of Medicine, Bahawal Victoria Hospital, Bahawalpur. Type of study was cross sectional. Hypoalbuminemia and mortality rate was assessed. Results: In present study, mean age of the patients was 49.78 ± 4.43 years. Out of 180 patients of acute ischemic stroke, hypoalbuminemia was found in 68 (38%) patients. Mortality rate was 10 (26%) and rest of the patients were alive. Hypoalbuminemia was found in 28 (28.28%) patients and 40 (49.38%) patients respectively in age groups 40-50 years and 51-60 years. Association of hypoalbuminemia with age group was significant (P = 0.005). Conclusion: Results of this study showed a higher rate of hypoalbuminemia and mortality in cases of acute ischemic stroke. Hypoalbuminemia was more common in 5th decade and significant association of hypoalbuminemia with age was noted. Most of the stroke patients were male and hypoalbuminemia was also common in male patients. Significant association of hypoalbuminemia with diabetes mellitus was observed. Introduction:Due to the high prevalence, disability, and cost incurred by acute ischemic stroke, several parameters to predict the functional outcome of stroke have been developed. Brain-Derived Neurotrophic Factor (BDNF) is a protein that plays an important role in neuroplasticity after a stroke attack. Lower BDNF level was associated with increased risk of developing stroke and poor prognostic functional outcome in stroke patients. Here, we measured serum BDNF levels in the acute and chronic phases of acute ischemic stroke patients to see whether the level was changing and affecting the functional outcome. Methods: A hospital-based prospective cohort study was conducted in the three largest governmental hospitals in Yogyakarta, Indonesia. Acute ischemic stroke patients were consecutively recruited fromJune 2018 until July 2019. Serum BDNF level measurements using enzyme-linked immunosorbent assay (ELISA) and functional outcome assessments using Barthel Index (BI) were performed on the 5th and 30th days after stroke onset, representing the acute and chronic phases of stroke, respectively. Results: Sixty-eight patients completed the study and were categorized into dependent (n=22) and independent (n=46) groups according to BI score on the 30th day after stroke onset. The mean serum BDNF level in the acute phase of the independent group was significantly higher than the dependent group (27,152.28 vs 23,143.41; p=0.044). Similar results were also found in the measurement of serum BDNF levels in the chronic phase in which the mean serum BDNF level of the independent group was found to be significantly higher than the dependent group (27,526.48 vs 22,818.91; p=0.036). There were no significant changes in the measurement of serum BDNF level between the acute and chronic phases in both dependent and independent groups. Conclusions: Serum BDNF level, either in the acute or chronic phase of stroke onset, may predict the functional outcome of the acute ischemic stroke patients.
