We describe a 56-year-old man with a 2-year history of papulonodules, pruritic and painful on palpation, on the head, trunk, limbs, buttocks and scrotum and a 1.5-year history of rheumatoid arthritis-like joint changes. Biopsies from the nodules on the head and left elbow revealed multinucleated giant cells with eosinophilic 'ground-glass' cytoplasm. Computed tomography revealed that there were scattered nodules in the liver and both lungs. Biopsies taken from nodules in the right lung and liver were consistent with multicentric reticulohistiocytosis. The widely scattered cutaneous papulonodules and the generalized systemic involvement make this patient interesting, and the condition should be differentiated from other diseases in clinicopathological practice.

The thirty-fourth case of multicentric reticulohistiocytosis with skin nodules and arthritis exhibiting typical histiocytic infiltrates in skin and synovium in a 16-year-old white female high school school student is presented. Unlike the usual reported course of this disease, the arthritis is currently of moderate degree and has been only slowly progressive. A brief review of the major manifestations of this disorder is given.Multicentric reticulohistiocytosis is a rare systemic disorder of unknown etiology manifested by the development of nodules containing infiltrates of characteristic histiocytic and multinucleated giant cells in synovium, skin, subcutaneous tissues and occasionally bone and other tissues. T h e hallmarks of the disease are arthritis which may be of a severe mutilating type, associated with skin and mucosal lesions which may become disfiguring, especially in the face. Although over a dozen names have been proposed, multicentric reticulohistiocytosis (MR) and the term lipoid dermotoarthritis have found the widest acceptance, the former becoming the more accepted of the two.In a recent critical review of the world literature, Barrow and Holubar (1) assembled a total of 33 cases of this disorder which met the criteria of both arthritis and skin lesions, though they noted that there were probably

Histological and electron microscopic examination has been carried out on the biopsies from a patient with histiocytosis gigantocellularis. The following findings have been observed. (1) The cells of histiocytosis gigantocellularis have characteristic granules of different size, form, and electron density, mostly showing a lamellar structure, as well as a membranous limit. These granules are probably synthetized within the cells. Their chemical composition and function are unknown. (2) Similarity between the electron-dense granules and those of Merkel’ cells, as well as the richness of unmyelinic nerve fibers in the immediate environment of the granuloma cells are discussed. Desmosomal structures have not been observed, but there are many cytoplasmic processes which are cogged in labyrinth-like fashion, partly forming a membranous complex. (3) The fine structure of the nucleolus, a well-developed endoplasmic reticulum, as well as a large amount of microfibrils and intracellularly localized collagen fibrils are features of fibroblasts, and may possibly be a transition stage of the granuloma cells. It is supposed that granuloma cells represents a degree of differentiation between reticulum cells and fibroblasts on the one hand and histiocytes (macrophages) on the other. Enlarged hollows are interpreted as a phagocytotic phenomenon of storing of lipids. (4) The nature of the stimulus leading to the formation of systematized giant cell granuloma is thus far unknown.
