A selected review of the literature and an empirical analysis of drug treatment compliance by schizophrenic patients

Abstract: One of the significant issues in the treatment of schizophrenic patients is their nonadherence to medication. While maintenance without medication may be possible for a minority of this group (Gardos and Cole, 1976; Cheung, 1981), most authorities believe that drug therapy should be continued indefinitely, particularly in view of the substantial risk of relapse when medication is discontinued (Prien and Klett, 1972). Davis (1975) reported that the evidence for the efficacy of neuroleptics in preventing relapse… Show more

“…Studies of Fitch et al and Huang et al, may explain this finding, which reported younger individuals constituting more inpatient visits compared to older individuals due to poor medication adherence. [ 2 , 21 ] Early medication nonadherence was found to be a predictor of poor drug adherence that leads to the pattern of disease relapse and hospitalization that is sustained throughout an individual's life, [ 39 , 40 ] which may explain the up-trending hospitalization rate for all standardized age groups. Furthermore, the decrease in hospitalization rate from younger to older age groups in every observed year could be due to low adherence in younger individuals.…”

“…The HBM hypothesizes that a person will generally not attempt to address a condition or illness unless they perceive themselves as potentially vulnerable to the condition, which must be threatening, and are convinced of the bene ts of treatment. A number of authors have shown the importance of the HBM in explaining compliance in medical settings (Adam & Howe, 1993;Chan, 1984;Pool & Elder, 1986).…”

“…Nonadherence to the treatment in schizophrenia has been shown to be the most important cause for relapse and readmission to the hospital. [ 8 ] Nonadherence can lead up to four to five time's higher chances of patients with schizophrenia to relapse. [ 9 ] Nonadherence has also been shown to be associated with increased rates of involuntary admissions, slower recovery and longer hospital stay,[ 10 ] higher risk of suicide,[ 11 ] poorer prognosis,[ 12 ] and low satisfaction with life.…”
