IntroductionAsthma is considered an important problem in children and influences on their everyday functioning. This study was concerned with the impact of asthma on the health‐related quality of life (HRQOL) of newly recognized pediatric asthma patients and their parents.MethodsPatients were identified by a general respiratory survey. HRQOL was assessed by self‐report measures: disease‐specific questionnaire for children, the Pediatric Asthma Quality of Life Questionnaire (PAQLQ) and generic for parents—the Quality of Life in Child's Chronic Disease Questionnaire (QLCCDQ).ResultsTwenty‐nine newly diagnosed asthmatic children aged 7–11 years (mean age = 8.6; SD = 1.4) were identified from a population‐based respiratory health survey. The most impaired HRQOL domains differed in parents and children. In children, activity limitation was the most impaired; however in parents, emotional functioning and symptom perception domains were most impaired. There were no statistically significant differences in patient and parental HRQOL observed in either the group where symptoms were present or in the group were symptoms were not present. Baseline spirometry showed correlation for FEV1%FVC and PAQLQ symptom domain (r = 0.42). Exercise test showed correlations between FEV1%FVC and QLCCDQ emotional domain (r = 0.7); MEF 25–75% and family roles (r = 0.81), as well as social functioning (r = 0.81). Activity limitation on the PAQLQ correlated with the MEF 25–75% (r = 0.44) and PEF (r = 0.55).ConclusionsHRQOL newly recognized children with asthma and their parents does not correlate closely with clinical status. This study aimed to investigate the effect of treating patients with moderate asthma with formoterol on quality of life (QoL) and to compare several questionnaires in their ability to detect changes in QoL. In an eight month multicentre, randomized, placebo-controlled trial, patients with asthma using daily inhaled corticosteroids and beta2-agonists (> or = 5 inhalations x week(-1)) were randomized to 6 months treatment with formoterol 24 microg b.i.d. (by Turbuhaler) or a matching placebo. Patients recorded symptoms (maximal score 21) and peak expiratory flow (PEF) twice daily. QoL was measured with two asthma-specific questionnaires, the Asthma Quality of Life Questionnaire (AQLQ) and the Living with Asthma Questionnaire (LWAQ), and with two generic QoL forms, i.e. the Short Form 36 (SF36) and the Psychological and General Well-Being scale (PGWB) at randomization and after 6 months of treatment. Out of 110 patients studied, 56 received formoterol and 54 placebo. Mean baseline forced expiratory volume in one second was 65% predicted. Mean morning PEF was 369 L x min(-1). Mean total symptom score was 3.6. Morning PEF increased (26.8 L x min(-1), p=0.0001) and symptoms decreased (-1.23, p=0.012) in the formoterol group in contrast to placebo. QoL improvements were measured in the LWAQ total score (baseline 0.61, change -0.05, p=0.048) and the physical construct of the LWAQ (baseline 0.71, change 0.07, p=0.044). The AQLQ and the generic QoL instruments showed no significant changes. In conclusion, the improvement in quality of life reported after 6 months was very small and only reflected by the Living with Asthma Questionnaire.

Today the most important challenge facing the pediatrician is the increasing prevalence of chronic diseases. With this regard, pediatricians play a key role in the management of these conditions. The closeness with the family, the knowledge of the clinical case and the care continuity allow the pediatrician to acquire a position of director of every case. When pathological events have a chronic feature, suddenly the quality of life of the whole family changes. For this reason the first communication of chronic disease is very important and the task of the pediatrician should be to provide a positive message to help the family in facing the difficulty of this new challenge. The bronchial asthma is the most common chronic disease worldwide. The incidence, the prevalence, and the mortality of the disease have increased in children over the past decades. These trends are particularly marked above all in preschool children. The success reached by Pediatricians is closely related to the compliance and the implementation of the therapy followed by the little patient and his family. With this regard authors, in this review, focus on the illustration of several strategies, based on the pediatrician' skills and medicine documents, that can be used for the improvement of communication among pediatrician-family and child, never forgetting the human aspect of the same doctor, that should conciliate with the scientific knowledge in the taking care of a specific chronic disease.
