Objective: An experiment was conducted to determine the effects of chanting the maha mantra on stress, depression, and the three gunas—sattva (enlightenment), rajas (passion), and tamas (inertia)—described in the Vedas as the basis of human psychology. Primary hypotheses of the study were that the maha mantra group would increase sattva and decrease stress, depression, rajas, and tamas significantly more than the other groups. Method: Participants were tested at pretest, posttest, and follow-up, with testing times separated by 4 weeks. Participants were randomly assigned to a maha mantra group, an alternate mantra (placebo) group, and a control group. Results: MANOVA results supported these hypotheses from pretest to posttest at p < .05 for all dependent variables except rajas. Conclusions: The authors suggest that the maha mantra has potential in addressing problems related to stress and depression and that it be considered as one possible component of a spiritual approach to social work practice.

PURPOSE Although spiritual care is a core element of palliative care, it remains unclear how this care is perceived and delivered at the end of life. We explored how clinicians and other health care workers understand and view spiritual care provided to dying patients and their family members. METHODSOur study was based on qualitative research using key informant interviews and editing analysis with 12 clinicians and other health care workers nominated as spiritual caregivers by dying patients and their family members.RESULTS Being present was a predominant theme, marked by physical proximity and intentionality, or the deliberate ideation and purposeful action of providing care that went beyond medical treatment. Opening eyes was the process by which caregivers became aware of their patient's life course and the individualized experience of their patient's current illness. Participants also described another course of action, which we termed cocreating, that was a mutual and fl uid activity between patients, family members, and caregivers. Cocreating began with an affi rmation of the patient's life experience and led to the generation of a wholistic care plan that focused on maintaining the patient's humanity and dignity. Time was both a facilitator and inhibitor of effective spiritual care.CONCLUSIONS Clinicians and other health care workers consider spiritual care at the end of life as a series of highly fl uid interpersonal processes in the context of mutually recognized human values and experiences, rather than a set of prescribed and proscribed roles.

AbrégéLes soins palliatifs visent à soulager la souffrance des patients atteints de Les soins palliatifs visent à soulager la souffrance des patients atteints de maladies incurables telles que le cancer; ils débutent lorsqu'il n'est plus possible de guérir ou de maîtriser activement la maladie (Rose, 1995). Le nombre de patients atteints d'une maladie incurable reste important puisque le taux d'incidence du cancer s'élève à cause du vieillissement de la population (Mount, Scott et Cohen, 1993). Il en ressort qu'on devra faire face, à l'avenir, à un nombre important de personnes atteintes d'une maladie terminale. Malgré les progrès réalisés sur le plan du traitement, 50 % des patients aiguillés vers les centres régionaux de cancérologie meurent encore de leur maladie (Roe, 1992). La qualité des soins prodigués aux patients en soins palliatifs représente un défi toujours plus grand à mesure que les coûts des soins de santé continuent d'augmenter et les budgets de rétrécir (Deachman, 1995).On reconnaît dans les soins palliatifs une option de soins communautaires viable étant donné que le nombre de lits d'hôpital est à la baisse et que davantage de patients choisissent de mourir chez eux. Du fait des progrès technologiques, les soignants naturels de patients atteints de cancer pourraient fort bien avoir besoin de soutien émotionnel et pratique pour répondre aux exigences toujours plus complexes des traitements (Silveira et Winstead-Fry, 1997). Cet article explore l'utilisation qui pourrait être faite de la théorie de Watson sur le caring humaniste en vue de répondre aux besoins des patients en soins palliatifs et de leurs proches. On démontrera en particulier l'utilisation des facteurs carative dans la prestation des soins palliatifs (voir le tableau un).On a examiné les écrits relatifs aux besoins des patients en soins palliatifs et de leurs proches. Pour les patients, on a dégagé les principales catégories suivantes: besoins physiques, psychosociaux, spirituels. Cela correspond aux besoins déterminés par l'Association canadienne des soins palliatifs [ACSP] (1995). De nombreux écrits portant sur les soins palliatifs considèrent que les membres de la famille et le patient constituent l'unité de soins, surtout lorsque le patient est soigné à domicile. Les besoins de la famille, tels que mentionnés dans les écrits, appartiennent aux catégories suivantes: besoins liés aux soins physiques du patient, besoins psychosociaux et besoins d'information.On a retenu la théorie du caring humaniste de Watson (1985, 1988) comme cadre pour la prestation des soins palliatifs à cause de sa compatibilité avec la philosophie du nursing en soins palliatifs. Roe (1992) expliquait que la satisfaction des besoins des patients en soins palliatifs exigeait un niveau particulier de connaissances et de compétences (la science), ainsi qu'un caring empreint de compassion (l'art).
