Introduction: Total thyroidectomy has been considered to be a treatment of choice for thyroid malignancies. It has recently gained popularity as gold standard for benign thyroid disorders requiring surgical treatment. Because of higher number of complications associated with total thyroidectomy, it is still considered an adventurous endeavor. Most important improvements in thyroid surgery include laparoscopic thyroidectomy, energy based devices (EBD) like Harmonic Focus® and LigaSure® for dissection and hemostasis, intraoperative neuromonitoring, and parathyroid hormone (PTH) assay technology. Aim of Study: Recent studies have suggested that despite lesser complication rates with LigaSure dissection method in total thyroidectomy, it is associated with prolonged operation time. Aim of our study was to compare conventional suture tie technique and LigaSure thyroidectomy, in terms of perioperative complications including bleeding, recurrent laryngeal nerve (RLN) injury, postoperative hematoma, hypocalcemia, operation time, and costeffectiveness.

Background Hypocalcemia is one of the most common complications of thyroidectomy, and vitamin D deficiency has been found to be an independent risk factor. Sweden is located north of the 55th latitude, resulting in a significant seasonal variation in sun exposure, thereby large variation in the naturally occurring levels of vitamin D. This study aimed to determine if there is a correlation between season of surgery and post-thyroidectomy hypocalcemia. Methods We conducted a retrospective register-based observation study on patients who had undergone total thyroidectomy during 2008–2015. In total, 7125 patients operated in Swedish facilities were identified via the Scandinavian Quality Register for Thyroid, Parathyroid, and Adrenal Surgery (SQRTPA). Patients operated during February–April were included in the dark group and patients operated during August–October were included in the bright group. Further stratification was made on the indication for surgery. The primary outcome was post-operative calcium treatment due to hypocalcemia, defined as having received calcium orally or intravenously before discharge. Results The risk of receiving post-operative calcium treatment was significantly lower in the bright group (29.7%) compared to the dark group (35.1%), with a relative risk of 0.846 (P < 0.001). This correlation held true if the indication for surgery was goiter or thyrotoxicosis. For malignancy, there was no significant difference between the groups. Conclusion In this cohort, total thyroidectomy performed during August–October was associated with a lower rate of calcium treatment given post-operatively when compared to total thyroidectomy performed during February–April. This would indicate a decreased risk of post-operative hypocalcemia if surgery was carried out after the brighter season.

Background: Thyroid surgery becomes challenging, due to closely related structures like recurrent laryngeal nerve and parathyroid gland. Microscope assisted surgery has better visualization for dissection, and decreases the risk of injury to the nerve, parathyroid gland, and its vascular pedicle. Hence it decreases the operative complications.Methods: This is a retrospective study of microscope-assisted thyroidectomy at Adichunchanagiri hospital and research centre, from May 2016 to April 2020. Ethical approval was obtained from institutional review committee. Types of surgery, post-operative hypocalcemia and recurrent laryngeal nerve function were analysed descriptively.Results: Out of total 30 microscope assisted thyroidectomy, 26 were female, 42 (87.5%) benign pathology, 6 (12.5%) malignant. Hemithyroidectomy was done in 25 (83.3%), total thyroidectomy 4 (13.3%), and completion thyroidectomy with neck dissection in 1 (3.3%). Among total thyroidectomies, transient hypocalcemia occurred in 2 (6.6%) and no temporary or permanent recurrent laryngeal nerve palsy.Conclusions: The use of microsurgical technique and loupes magnification in thyroid surgery are safety and effective procedures that require an appropriate training in microsurgery, but may significantly reduce post-operative complications.
