In this study, we aimed to compare the efficiency of electrocautery with the LigaSure™ vessel sealing system. Patients and methods:This randomized study included a total of 53 patients (44 males, 9 females; mean age: 63.7±8.4 years; range, 39 to 83 years) who underwent coronary artery bypass grafting (CABG) for radial artery (RA) harvesting with electrocautery or LigaSure™ vessel sealing system between January 2019 and September 2019. The patients were randomly assigned into two groups. Group 1 consisted of 27 patients operated with the LigaSure™ small jaw sealer/divider and Group 2 consisted of 26 patients operated with electrocautery. The length of the conduit, the amount of intraoperative blood loss, the number of hemostatic clips before and after the anastomosis, the amount of postoperative blood loss, the number of ecchymoses, hematoma and infection are compared. Results:The amount of blood loss from the beginning of harvesting to the end of the closure of the skin of left arm was significantly lower in Group 1 (p=0.004). The difference in the number of hemostatic clips before and after the anastomosis of RA to coronary artery was statistically significantly lower in Group 1 (p<0.001 and p<0.001, respectively). There was no statistically significant difference in the amount of blood loss postoperatively (p>0.6), number of ecchymoses (p>0.1), and postoperative hematoma (p>0.1) and infection (p>0.03) between the groups. Conclusion:Our study results suggest that the LigaSure™ vessel sealing system is a better option of RA harvesting, particularly in patients who are prone to bleed.

Background: In thyroid surgery, achieving accurate haemostasis is fundamental in order to avoid the occurrence of complications. Energy-based devices are currently extensively utilized in this field of surgery. This study aims to compare Harmonic Focus and Thunderbeat Open Fine Jaw with regard to surgical outcomes and complications. Methods: Patients submitted to total thyroidectomy in our center, between January 2017 and June 2020, were retrospectively analysed. Based on the energy-based device utilized, two groups were identified: Group A (Harmonic Focus) and Group B (Thunderbeat Open Fine Jaw). Results: A total of 527 patients were included: 409 in Group A and 118 in Group B. About surgical outcomes, the mean operative time was significantly shorter in Group B than in Group A (p < 0.001), while as regards complications, the occurrence of transient recurrent laryngeal nerve injury was significantly greater in Group B than in Group A (p = 0.019). Conclusions. Both Harmonic Focus and Thunderbeat Open Fine Jaw have proven to be effective devices. Operative times were significantly shorter in thyroidectomies performed with Thunderbeat Open Fine Jaw; however, the occurrence of transient recurrent laryngeal nerve injury was significantly greater in patients operated on with this device.

Background: Remarkable increase in incidence of thyroid cancer over previous decade has resulted in increased need for thyroid surgery. Aim and objectives: Goal of research was to evaluate safety and efficacy of using vessel sealing device ligasure (COVIDIEN®) in terms of intraoperative and postoperative complications, comparing to standard vascular ligation in open thyroid surgery. Patients and methods: A randomized-controlled trials that was conducted at Department of General Surgery of Assiut University Hospitals. A total of 80 patients were scheduled for thyroidectomy participated in the research. Those patients were randomly subdivided into 2 categories. Category 1 included cases who underwent conventional vessel ligation (n= 40 patient) and category 2 included cases who underwent ligasure vessel sealing (n=40 patients). The study was conducted in the period between 2019 and 2021. Results: Both groups had insignificant difference regarding overall recorded complications (8 (20%) vs. 7 (17.5%); p= 0.51). One patient in each group developed hoarseness while recurrent transient laryngeal nerve damage was recorded in two patients in each category. The most frequent complication was transient hypocalcaemia (12.5% of conventional ligation and 10% of ligasure sealing group). Conclusion:Ligature was superior to conventional approach in terms of lowering operation time, perioperative and postoperative blood loss, and parathyroid damage. RLN injuries, on the other hand, were comparable in both categories. Although shorter operative periods led to lower operating room occupancy expenses, overall cost of surgery was considerably greater in ligasure category.
