A retrospective multicentre study on the evaluation of perioperative outcomes of single‐port robotic cholecystectomy comparing the Xi and SP versions of the da Vinci robotic surgical system

Abstract: Background: Single-incision robotic cholecystectomy (SIRC) is widely performed with both the da Vinci Xi system (Xi) and the da Vinci SP system (SP). But there are limited numbers of studies comparing these platforms. Methods: Patients who underwent SIRC between 2019 and 2020 were enrolled.Patient demographics, intraoperative factors, postoperative complications, postoperative pain were compared using a one-to-one propensity score matching (PSM).Results: Overall, 258 patients underwent SIRC with Xi and 72 with … Show more

“…Moreover, some of them require urgent surgery, for which robotic cholecystectomy is difficult to perform due to scheduling and administrative issues. The case selection in our study aligned with those in two previous studies conducted in Korea 3 , 4 . It is always a concern before robotic surgery in Korea.…”

“…Almost all the procedures were successfully performed, except in one case with acute cholecystitis, which was converted to open surgery. In the study by Kang et al, significantly more cholecystectomies were also attempted in patients with acute cholecystitis using the SP than the Xi system (31.9% vs. 1.6%; P < 0.001) 4 , which might indicate that acute inflammation or disease severity affected the surgeons’ preference for a specific type of robotic system when performing robotic cholecystectomy. However, when deciding to perform robotic surgery, in addition to the safety of the operation, the medical costs of robotic surgery should not be ignored.…”

“…Patients without personal insurance usually cannot undergo robotic surgeries. This case selection was similar to that in two previous studies in Korea (3,4). Moreover, for this reason, patients who have undergone previous upper abdominal surgeries or preoperative ERCP or were suspected of having severe cholecystitis were generally not considered for robotic surgery because adhesion and in ammation might increase the risk of conversion to laparoscopic or open surgery.…”
