Zusammenfassung Gegenstand und Ziel Die Nutzung intravenöser Verweilkatheter ist trotz größter Sorgfalt mit Komplikationen verbunden. Gewebeveränderungen und minimale Entzündungsreaktionen an der Vene können bereits vor klinisch erkennbaren Veränderungen auftreten. Ziel dieser Studie war es, die Vena jugularis während und nach der Katheterisierung klinisch und ultrasonografisch zu beurteilen und eine mögliche bakterielle Besiedlung des Katheters zu evaluieren. Material und Methoden In einer prospektiven Beobachtungsstudie wurden 55 Pferde mit einem Venenverweilkatheter täglich klinisch und ultrasonografisch untersucht. Die steril entfernte Katheterspitze wurde mikrobiologisch mittels Anzucht sowie morphologisch in der Rasterelektronenmikroskopie untersucht. Die erhobenen Daten wurden deskriptiv analysiert und die Venenwanddicke an verschiedenen Lokalisationen und Zeitpunkten hinsichtlich verschiedener Parameter mit nicht-parametrischen Tests verglichen (Signifikanzniveau p < 0,05; Bonferroni-Korrektur bei multiplem Testen). Ergebnisse Die Verweildauer des Katheters betrug zwischen 19 Stunden und 10 Tagen (Median 69,5 Stunden). Es konnten keine behandlungswürdigen Venenerkrankungen beobachtet werden, lediglich transiente geringgradige Gewebeveränderungen traten bei 41,8 % der Patienten auf. Im Ultraschall konnten Venenklappen und Kollateralgefäße zuverlässig dargestellt werden. Lokalisation (p ≤ 0,001), Jahreszeit (p = 0,006) und Allgemeinanästhesie (p ≤ 0,001) hatten einen signifikanten Einfluss auf die im Ultraschall gemessene Venenwanddicke, welche an der Einstichstelle verbreitert war. Der Nachweis von Bakterien war positiv in 23,5 % (12/51 Proben, mikrobiologische Anzucht) bzw. 25,0 % (4/16 Proben, Rasterelektronenmikroskopie). Schlussfolgerung und klinische Relevanz Jeder Katheter erzeugt eine Entzündungsreaktion der Venenwand und des umliegenden Gewebes. Diese Veränderungen können vor allem in der ultrasonografischen Untersuchung dargestellt werden, welche eine praxistaugliche Diagnostik darstellt. Eine Vorhersage über sich entwickelnde Venenerkrankungen war bei den untersuchten Probanden nicht möglich. Die Ergebnisse der Rasterelektronenmikroskopie zeigen, dass nicht in jedem Fall morphologisch auf dem Katheter erkennbare Bakterien auch in der Kultivierung nachgewiesen werden können.

Tromboflebite jugular em equinos submetidos à laparotomia para o tratamento de afecção gastrointestinal

RESUMO.-Afecções do trato gastrointestinal de equinos podem causar distúrbios de coagulação devido à concentração elevada de mediadores inflamatórios que estimulam a hemostasia e inibem a fibrinólise, gerando um estado de hipercoagulação. Equinos submetidos à laparotomia no tratamento da síndrome cólica permanecem com cateter venoso durante vários dias para a administração de fluidos e fármacos no período pós-operatório e, a veia jugular é o principal local para a implantação de cateteres. Assim, o trauma vascular persistente causado pelo cateter, associado ao ambiente pró-trombótico induzido pela afecção gastrointestinal, aumenta o risco para o desenvolvimento de tromboflebite jugular. Objetivou-se avaliar as características físicas e ultrassonográficas da veia jugular canulada com cateter de poliuretano durante o período pós-operatório de equinos submetidos à laparotomia. O cateter foi inserido de forma asséptica à admissão e permaneceu por sete dias. A avaliação ultrassonográfica revelou o desenvolvimento de tromboflebite em um equino, 48 após o procedimento cirúrgico. Os demais equinos demonstraram espessamento da parede vascular no local de punção e pequenos trombos junto ao cateter. A monitoração ultrassonográfica demonstrou que a cateterização prolongada em equinos submetidos à Coagulation abnormalities are usually associated with equine gastrointestinal disease due to the increased levels of inflammatory mediators, which promotes hemostasis and inhibit fibrinolysis, creating a hypercoagulable state. Horses underwent laparotomy to treat colic usually require a venous catheter for several days to administrate fluids and drugs during the postoperative period, and the jugular vein is the most frequent site for catheterization. Therefore, the persistent vascular trauma caused by an implanted catheter, associated with the prothrombotic environment induced by the gastrointestinal disorder, increases the risk for the development of jugular thrombophlebitis. The purpose of the present investigation was to evaluate physical and ultrassonographic features of the jugular vein cannulated with a polyurethane catheter during the postoperative period of horses underwent colic surgery. The catheter was inserted aseptically on admission and dwell time was seven days. Upon ultrasound examination, one horse developed thrombophlebitis 48 hours after surgery and the other horses showed thickened venous wall at puncture site and small clots associated to the catheter. Ultrasound monitoration showed that long-term catheterization in horses underwent colic surgery following the present protocol minimizes vascular trauma and could prevent jugular thrombophlebitis.

The purpose of the present investigation was to examine the effects of unilateral and bilateral jugular vein occlusion via temporary surgical ligature on arterial blood gases in horses during treadmill exercise. Six horses performed three exercise tests (ETs). ET1, considered to be the control, was performed in horses without jugular occlusion. ET2 and ET3 were performed in horses with unilateral and bilateral occlusion via temporary surgical ligature of the jugular veins, respectively. The partial pressure of oxygen (PO 2 ) and partial pressure of carbon dioxide (PCO 2 ) were determined. The PO 2 showed decreased values during ET2 and ET3, suggesting that horses presenting acute jugular thrombophlebitis may have airflow limitations when exercising. Key words: Blood gases, equine, swan ganz, thrombophlebitis, upper airways ResumoO objetivo do presente trabalho foi avaliar os efeitos da oclusão da veia jugular por ligadura cirúrgica unilateral e bilateral sobre os gases do sangue arterial em equinos durante o exercício em esteira. Seis equinos realizaram três exercícios teste (ETs). ET1, considerado como controle, foi realizado por equinos sem oclusão jugular. ET2 e ET3 foram cumpridos por equinos apresentando oclusão da veia jugular por ligadura temporária unilateral e bilateral, respectivamente. A pressão parcial de oxigênio (PO 2 ) e a pressão parcial de dióxido de carbono (PCO 2 ) foram determinadas. A (PO 2 ) demonstrou diminuição em ET2 e ET3, sugerindo que equinos com tromboflebite jugular aguda podem apresentar limitação no fluxo de ar inspirado durante o exercício.
