A reappraisal of intra-arterial chemotherapy.Results obtained in 145 patients with head and neck cancer treated during 1963-1966 with intra-arterial chemotherapy followed by radical radiotherapy

In eight cases of extended tumors of oral cavity, we have studied the growth rate, the cell kinetics parameters, and the histology before and during treatment. We have found that growth rate and cell kinetics are not useful for clinical purposes because the first is not related to the response to treatment, while the second is unable to give relevant parameters in solid human tumors. New information was obtained from examination of serial biopsies before and throughout the treatment. These data suggest that multiple subpopulation groups of cancer cells with different intrinsic sensitivity to chemotherapy and/or radiotherapy may exist in the same tumor. Resistance of the tumor is related to some groups of cells scarcely sensitive to treatment.

This paper presents the results of a controlled therapeutic trial comparing the combination of intra‐arterial infusion chemotherapy and radiotherapy with radiotherapy alone in advanced (T4) cancers of the oral cavity and oropharynx. Tumor regression was far greater with the combination treatment: 40% compared to 10% with radiotherapy alone. The median survival rate was 6 months in the group receiving radiotherapy alone, and 9 months in those receiving the combination. While recognizing the importance of the treatment, the authors realize the necessity of continued investigation into different combination possibilities in order to obtain better survival results.

Fifteen patients were treated with intra-arterial fluorouracil, methotrexate, and bleomycin in combination for palliation of advanced cancer of the head and neck. In all cases irradiation or irradiation plus surgical resection had previously failed to control the tumor. Tumor regression occured in 87% of the cases (13/15), and was complete in three (20%). Regressions lasted for up to 13 months. In four instances tumor regression permitted radical resection. The response rate achieved with this drug combination was greater than that observed earlier using each drug individually. Intra-arterial chemotherapy provides useful palliation for advanced head and neck cancer and the results can be improved with drug combinations.
