A rare case of hyperoxaluria presenting with acute liver injury and stone-free kidney injury

Abstract: A 49-year-old woman visited the clinic because of acute hepatitis and acute kidney injury with decreased urine output presenting microscopic hematuria and proteinuria. An abdominal computed tomography revealed a localized, hypoattenuated lesion in a hepatic lateral segment, and kidney biopsy showed oxalate crystal deposition with tubular necrosis. In addition, the patient׳s 24-hour urinary excretion of oxalate was increased. Her kidney and liver injury improved after sessions of hemodialysis, and urinary oxala… Show more

“…The prevalence of PH1 is higher in Mediterranean consanguineous populations, and the proportion of PH1 as a cause of pediatric ESRD is 10% in Kuwait 13) and 13% in Tunisia 14) . There have been no data on large cohorts except a few case reports of PH in Korea so far 6,7) . Although we did not analyze the prevalence of PH1, given that the patients in our study were limited to pediatric cases from three tertiary centers in Korea with a total population of approximately 50 million, the prevalence of PH1 in Korea might be similar to that in Western coun tries.…”

“…Although there have been several case reports of PH in Korea 6,7) , systematic studies have not been reported yet. Herein we report 11 cases of Korean patients with PH, with retrospective analyses of genotypephenotype correlations and the outcome of combined liver and kidney transplan tation in five patients.…”

“…Defects in liver enzymes involved in glyoxylate metabolism lead to oxalate overproduction and are the common cause of primary hyperoxaluria [ 85 , 86 ]. Hence, liver injury is associated with primary hyperoxaluria [ 87 , 88 ]. Secondary hyperoxaluria is caused by many factors, such as ingestion of high oxalate and oxalate precursor–rich foods and increased intestinal absorption of oxalate due to malabsorption syndrome [ 85 , 86 ].…”
