A Randomized Study Assessing the Systematic Search for Maxillary Sinusitis in Nasotracheally Mechanically Ventilated Patients

Abstract: The objective of this randomized study was to compare the occurrence of nosocomial pneumonia in nasotracheally intubated patients who were randomly allocated either to a systematic search of sinusitis by CT scan (study group) or not (control group). A total of 399 patients were included: 272 male and 127 female; mean age, 61 +/- 17 yr; SAPS: 12.6 +/- 4.9. The study group consisted of 199 patients and the control group consisted of 200. In the study group, sinus CT scans were performed in case of fever at Days … Show more

“…A standard treatment protocol was used to ensure that no known confounding factors would influence the difference in frequency of VAP between the two study groups. Specifically, all participants were orally intubated [13], placed in a 30-45˚semi-recumbent position [14] and received the same antibiotic and H2 receptor antagonists treatment regimen [25,26].…”

“…Patients were orally intubated [13], placed on the ICU bed in a semi-recumbent position (30-45˚) [14] and tracheostomy was not performed until completion of 3 weeks of MV. No closed circuits for tracheal suctioning were used [15].…”

“…Orotracheal and orogastric intubation have been associated with reduced incidences of VAP and nosocomial sinusitis [49,50]. Therefore, the oral route is preferred when intubation of the trachea and esophagus is necessary in a critically ill patient.…”

“…La intubación naso-traqueal no está exenta de esta situación, más aún, ésta se asocia a mayor incidencia de sinusitis 41,42 y ésta a su vez podría aumentar el riesgo de NAVM 41,43 . Un estudio controlado con un grupo de pacientes intubado por vía naso-traqueal y otro por vía oro-traqueal encontró en el análisis multivariado que la sinusitis es un factor que aumenta en 3,8 veces la frecuencia de NAVM 42 .…”
