A Randomized Controlled Trial Evaluating the Active Communication Education Program for Older People with Hearing Impairment

Abstract: This research study provides evidence for the effectiveness of the ACE program and indicates that such communication programs have an important place in the audiological rehabilitation of older adults. They should be considered as an alternative or a supplement to traditional interventions such as hearing aid fitting.

“…Services might include providing guidance to the person with hearing loss and their family members concerning the psychosocial and communicative effects of hearing loss, strategies to function in difficult listening situations, and an orientation to assist in the adaptation to hearing aids. Studies designed to test the use of aural rehabilitation, as an alternative or supplement to hearing aid fittings, to improve communication and quality of life in older adults with hearing loss need to be conducted (20,26).…”

“…The program was expanded based on the results and experiences drawn from Study III, with inspiration and some parts drawn from the program developed by Hickson and colleagues (Hickson et al, 2006;Hickson et al, 2007, Öberg et al, in press) and also with information regarding psychosocial-related situations, such as cognition, memory and applied relaxation tasks (Johansson et al, 2012;Pichora-Fuller, 2007). Hickson et al (2007) The participants in the control group were on a waiting list; in the meantime, material about the history of hearing aids (Nielsen, 2008) was sent for them to read. Three months after T1, the participants had to fill out the same self-reports online for a third time.…”

“…Many hearing-aid users have further and persistent hearing problems after rehabilitation, including difficulties knowing what actions to take to address problems (Popelka et al, 1998;Southall et al, 2006). Hickson and colleagues have shown that people either stop using their hearing aids or continue searching for more help and guidance (Hickson et al, 2007). For many hearing-aid users, it is essential to obtain further and extended rehabilitation to become satisfied with their hearing aids and overcome participation restrictions and activity limitations (Northern and Beyer, 1999;Sanders, 1971).…”

“…In the currently recommended nomenclature found in World Health Organization (2002) International Classification of Functioning, Disability and Health recommendations, the concepts earlier embodied in "handicap" now are termed participation restrictions and the concepts formerly known as "disability" are now termed activity limitations. In contemporary terms, Hickson, Worrall, and Scarinci (2007) characterized SAC as a measure of communication activity limitation. Although also loaded with communication activity limitation questions, SAC-Hx is by its nature an inventory of multiple domains, and as such, assesses both participation restriction and activity limitation.…”
