A randomized clinical trial of high volume peritoneal dialysis versus extended daily hemodialysis for acute kidney injury patients

Abstract: Despite faster metabolic control and higher dialysis dose and ultrafiltration with EHD, this study provides no evidence of a survival benefit of EHD compared with HVPD. The limitations of this study were that the results were not presented according to the intention to treat and it did not control other supportive management strategies as nutrition support and timing of dialysis initiation that might influence outcomes in AKI.

“…In another prospective study comparing the effect of HVPD and prolonged HD (PHD) on AKI patients' outcome, 51 the PHD and HVPD groups were similar in sex, disease severity scores, and etiology of AKI. There was a trend toward statistical difference regarding the presence of sepsis (62.3% in the PHD group vs 44.9% in the HVPD group, P=0.054).…”

“…Ponce et al [17], performed a trial to assess dosing patterns of PD in critically ill AKI patients, randomized to receive higher or lower intensity PD therapy (prescribed weekly Kt/V of 5.6 vs. 3.5). The 2 groups had similar mortality rates after 30 days (55 vs. 53%, p = 0.83).…”

“…The same Brazilian group performed another prospective study comparing the effect of HVPD and prolonged HD (PHD) on AKI patients' outcome [20]. Delivered Kt/V and ultrafiltration were higher in the PHD group; however, there was no difference between the 2 groups in mortality and recovery of kidney function or need for chronic dialysis.…”

“…The use of peritoneal dialysis may facilitate access to dialysis in some parts of the world. Several studies mainly from Brazil have shown that peritoneal dialysis can provide similar outcomes to other dialysis modalities in AKI [43,44,45]. …”
