A randomised controlled study of risperidone and olanzapine for schizophrenic patients with neuroleptic-induced acute dystonia or parkinsonism

Abstract: There is convincing evidence that environmental stress plays a significant role in modifying both mental and physical health. The biological mechanisms linking stress to ill health are not fully understood, but significant evidence points to a central role of the stress axes; the hypothalamic–pituitary–adrenal (HPA) axis and the sympathetic nervous system. Together these two systems link the brain and the body and are crucial in maintaining homeostasis as well as improving an organism’s survival chances in the… Show more

“…Total mean weight gain tended to be greater in studies with longer periods of follow-up compared with studies of shorter duration 24,66,70,74. However, the average rate of weight gain over the whole study period was greater in the studies of shorter duration, suggesting that the greatest weight gain may occur in the initial stages of olanzapine therapy 66,70,74. In comparative studies, olanzapine was generally associated with greater levels of weight gain than other antipsychotics 21,36,38,41,43,56,60,61,64,66,67,7375,77,80…”

“…Cortisol has been demonstrated to be a mortality indicator in patients with depression ( Chan et al, 2010 ). An exhausted cortisol production might lead to a hyper-immune state as acute immune responses continue unopposed ( McEwen, 1998 ).…”

“…Once a patient is on a stable dose of antipsychotic and anticholinergic, gradual withdrawal of the anticholinergic may be possible, though a quarter of patients may require reinstatement. For patients who have dystonias on a high‐potency antipsychotic, switching to a lower potency antipsychotic may reduce the risk of dystonias as well as parkinsonism.…”
