The Princeton Consensus (Expert Panel) Conference is a multispecialty collaborative tradition dedicated to optimizing sexual function and preserving cardiovascular health. The third Princeton Consensus met November 8 to 10, 2010, and had 2 primary objectives. The first objective focused on the evaluation and management of cardiovascular risk in men with erectile dysfunction (ED) and no known cardiovascular disease (CVD), with particular emphasis on identification of men with ED who may require additional cardiologic work-up. The second objective focused on reevaluation and modification of previous recommendations for evaluation of cardiac risk associated with sexual activity in men with known CVD. The Panel's recommendations build on those developed during the first and second Princeton Consensus Conferences, first emphasizing the use of exercise ability and stress testing to ensure that each man's cardiovascular health is consistent with the physical demands of sexual activity before prescribing treatment for ED, and second highlighting the link between ED and CVD, which may be asymptomatic and may benefit from cardiovascular risk reduction. The first conference convened in June 1999 to develop recommendations for clinical management of sexual dysfunction in men and women with known cardiovascular disease (CVD). This conference also provided a multidisciplinary forum for evaluation of the potential cardiovascular risk posed by sexual activity in atrisk patients. The first Princeton Consensus Conference recommendations 1 included stratification of patients by level of cardiac risk associated with sexual activity based on existing CVD. Those at low risk could initiate or resume sexual activity and be treated for sexual dysfunction. For those at high risk, sexual activity was deferred until the cardiac condition was stabilized. The second Princeton Consensus Conference convened in June 2004 and expanded the recommendations of the first conference to emphasize risk factor evaluation and lifestyle management for all men with erectile dysfunction (ED). 2 The second conference recommendations also incorporated new information on the appropriate use of phosphodiesterase type 5 (PDE5) inhibitors in men with ED and concomitant CVD.The third Princeton Consensus Conference took place November 8 to 10, 2010, in Miami Beach, Florida. The group revisited and updated their 2005 recommendations regarding the cardiovascular risk associated with sexual activity in men with known CVD. 2 In addition, the third conference focused on the predictive value of vasculogenic ED in assigning cardiovascular risk in men of all ages, with the primary objective being development of an approach to cardiovascular risk assessment in younger men with ED and no known CVD. 293Артериальная гипертензия (АГ) и ишемическая бо-лезнь сердца (ИБС) являются наиболее частыми за-болеваниями взрослого населения и характеризуют-ся высокой степенью риска преждевременной смер-ти [1]. По мнению экспертов ВОЗ остеопороз также при-знан одним из наиболее распространенных заболе-ваний, которое наряду с инфарктом миокарда, ин-сультом, онкологическими заболеваниями и внезап-ной смертью занимает ведущее место в структуре за-болеваемости и смертности населения [2]. По мнению экспертов Международного фонда остеопороза дан-ное заболевание приобретает масштабы эпидемии и так же, как и ССЗ, может быть отнесен к числу соци-ально значимых проблем здравоохранения многих стран [3]. Цель. Оценить влияние небиволола на минеральную плотность кости (МПК) у женщин постменопаузального периода с мягкой артериальной гипертонией (АГ) и ос-теопенией. Материал и методы. В рандомизированное контролируемое исследование были включены 56 женщин постменопаузального периода (по 28 пациентов в основной и контрольной группе) в возрасте от 50 до 65 лет с остеопенией и мягкой АГ. В течение 12 мес пациентки основной группы получали лечение небивололом от 5 до 7,5 мг/сут, а пациентки контрольной группы -атенололом от 12,5 до 25 мг/сут. Всем пациенткам до начала исследования и через 12 мес лечения проводили клиниче-ский осмотр, антропометрическое обследование, измерение артериального давления, регистрацию ЭКГ. Количественное определение МПК проводили методом двух-энергетической рентгеновской абсорбциометрии на остеоденситометре DELPHI W фирмы HOLOGIC (США) в поясничном отделе позвоночника (L1-L4), шейке бедра и проксимальном отделе бедренной кости в передне-задней проекции. Кроме того, определяли показатели кальциевого обмена и костного метаболизма: ионизиро-ванный кальций, общую щелочную фосфатазу, С-телопептид коллагена I типа (СТХ). Результаты. На фоне терапии мягкой АГ небивололом в течение 12 мес выявлено увеличение МПК в позвоночнике по Т-критерию с -1,7±0,4 SD до -1,4±0,53 SD (р<0,001), в то время как в группе терапии атенололом показатель уменьшился с -1,5±0,7 SD до -1,6±0,64 SD (p<0,001). При оценке Т-критерия шейки бедра в основной груп-пе его значение за время терапии изменилось с -1,4±0,44 SD до -1,27±0,5 SD (р=0,015), в контрольной с -1,3±0,64 SD до -1,5±0,65 SD (р=0,0005). На фоне ан-тигипертензивной терапии в основной группе значение Т-критерия проксимального отдела бедренной кости изменилось с -0,58±0,4 SD до -0,49±0,4 SD (р=0,003), в контрольной с -0,8±0,84 SD до -0,83±0,93 SD (р=0,3). Динамика МПК во всех исследуемых отделах на фоне терапии в течение 12 мес значимо различалась меж-ду основной и контрольной группами. На фоне терапии небивололом выявлено снижение уровня CTX в сыворотке крови с 0,367±0,16 нг/мл до 0,294±0,12 нг/мл (р<0,001), в то время как в контрольной группе отмечено повышение этого показателя с 0,369±0,15 нг/мл до 0,499±0,18 нг/мл(р<0,001). Objective: Behçet's disease (BD) is a systemic vasculitis, capable of involving all types of vessels. Endothelial dysfunction (ED) has been previously documented in BD. Previous studies showed that nebivolol might improve endothelial functions in endothelial dysfunction. The aim of our study is to assess the effects of nebivolol on endothelial dysfunction in patients with Behçet's disease. Methods: This study was designed as prospective single-arm controlled study. We prospectively studied 35 Behçet's patients who were diagnosed according to the International Study Group criteria. Patients received 5 mg nebivolol per day for 3 months. Endothelial dysfunction was evaluated by brachial artery flow-mediated dilatation (FMD) method using high-resolution vascular ultrasound device at baseline and after for 3-month therapy. The paired samples t test, Wilcoxon test, Pearson, Spearman correlation analyses were used for statistical analysis. Results: A significant improvement was observed in FMD after therapy period (4.23±1.19 vs 7.95±2.21%, p<0.001). The correlation analysis showed a negative correlation between post-treatment high-sensitive C-reactive protein and FMD (r=-0.435, p=0.009). No adverse effects were observed in treatment period. Conclusion: Nebivolol improved endothelial dysfunction in Behçet's patients. However, further comprehensive studies are needed to determine the long-term effects of nebivolol.
