Introduction: Multiple sclerosis (MS) can have negative consequences on caregivers’ level of performance and adaptive process. This study aimed to (i) describe the steps of developing an occupation-based program based on the Occupational Adaptation Model (OAM), and (ii) make a preliminary assessment of the feasibility of the program prepared for caregivers of people with MS. Method: In a mixed-method approach, the occupation-based program of “Adapting to the Occupational Challenges of Caregivers of people with MS” (AOCCMS) was developed in four steps. During Steps 1, 2, and 3 the AOCCMS program was developed and in step 4, the feasibility and acceptability of the program were evaluated. Results: Based on the results of step 1 that have been published in the past; time limitations in occupation implementation; care needs in occupation implementation; psychological reactions affecting occupations were occupational challenges affecting the daily occupations of caregivers. Also, occupational adaptation strategies were strategies to alleviate intrapersonal challenges of occupational adaptation and strategies to alleviate environmental challenges of occupational adaptation. In step 2, the intervention programs of studies on caregivers were reviewed. During step 3, the data obtained from steps 1 and 2 were merged and integrated. Finally, the AOCCMS program was developed. In step 4, during the feasibility and acceptability of the program, 75% caregivers were satisfied with the program content. The quality of the program content was excellent and very good for 100%. Conclusion: The AOCCMS is an occupation-based program which was evaluated to be feasible and acceptable, which may be further addressed in future research and clinical practice.

Background: Balance assessment is a very important part of rehabilitation for people with unilateral ankle sprain. Biodex balance analyzer is one of the devices used for assessing balance. Using oriented movements is a method of this device, thus we decided to compare these movements in both healthy and unhealthy feet. Methods: This article was a cross sectional study. In this study 35 men and women aged from 19 - 34 with unilateral ankle sprain were participated. First they stood on one leg on the Biodex device with open eyes and were then asked to look at the screen in front of them and move the cursor to specific points on the screen and with due to attention to the time and overall points, the device allocated a number to each foot, which a shorter time and straighter path leaded to better results. Results: The results showed a significant difference between the healthy and injured legs of the participants in terms of the overall score of the stability index, especially, in the anterior, medial and lateral points (P < 0.05). Conclusions: According to the results, that the goal-oriented movements in the anterior, medial, and lateral directions in the injured leg are significantly different from the healthy leg, it can be assumed that these directions are more affected in the injured leg. Therefore, it is recommended to pay attention to the injured directions in the retraining and rehabilitation programs of patients with lateral ankle sprains.

Background: The International Classification of Functioning, Disability, and Health (ICF) can identify body function, structure impairment, activity limitation, participation restriction, and contextual factors. Objectives: Considering the importance of the role of environmental barriers in disability and function, this study aimed to investigate the environmental barriers affecting the function of individuals with multiple sclerosis (MS) based on the Comprehensive ICF Core Set for MS. Methods: This cross-sectional study was conducted on individuals with MS who are registered in the ICF Medical Commission. The participants completed the consent form, the Persian version of the Craig Hospital Inventory of Environmental Factors, Functional Independence Measure (FIM), and a demographic questionnaire based on the ICF Core Set for MS. The data were analyzed using SPSS software (version 25). Results: A total of 140 individuals with MS (34 males and 106 females) participated in this study. The most frequently reported environmental barriers were structural and physical, and the least frequently reported barriers were related to work and school. The barriers related to attitudes and support were significantly associated with the motor function (P = 0.003) and total FIM score (P = 0.002). The variables of attitudes and support and policies were the main predictors of motor function (RS = 0.093, P < 0.001) and the total FIM score (RS = 0.109, P < 0.001), respectively. None of the barriers was significant and predictive of cognitive function. Conclusions: Since environmental barriers can affect the function of MS patients, authorities should implement the necessary strategies to eliminate these barriers and facilitate participation.
