A Prospective Randomized Study Comparing Hysteroscopy and Curettage (H & C) under Local Anaesthesia (LA) and General Anaesthesia (GA) in Chinese Population

Abstract: Hysteroscopy and curettage under LA and GA are equally acceptable in the Chinese population in Hong Kong. The patient satisfaction rate is high in both groups. Hysteroscopic diagnosis is highly accurate in malignant condition (100% sensitivity and 83% specificity).

“…This study compared prospectively the histologic findings from blinded dilatation and curettage and hysteroscopic biopsy. There are several studies that showed more accurate diagnosis of endometrial pathology with hysteroscopic biopsy than dilatation and curettage, but they were retrospective studies, 6 had various indication for procedure including premenopausal women, 7,8 used gross finding from hysteroscopy only, 9 or were compared with findings from ultrasonography. 3,4,10 Recently, there is a report that dilatation and curettage is an inadequate diagnostic tool for uterine focal lesions even though the detective accuracy of endometrial hyperplasia and carcinoma is relatively high.…”

“…Hysteroscopy has been advocated by many as the standard for the diagnosis of abnormal uterine bleeding (51). However, results of various studies are conflicting whether this method improves upon the sensitivity of D & C in the detection of endometrial hyperplasia or carcinoma (52–55).…”

“…Awake propofol plasma concentration calculated by Diprifusor among three groups was in range of 1.6–1.8 µg/mL which might help to predict the time to wake after discontinuation of propofol infusion. Although there has been a report for the success of hysteroscope and curettage under local anesthesia, 9 this intravenous‐based technique may be suitable for the patient who wants to sleep during the procedure and may avoid the undesired surgical condition.…”
