A prospective randomized study comparing suture mesh fixation versus tacker mesh fixation for laparoscopic repair of incisional and ventral hernias

Abstract: Suture fixation is a cost-effective alternative to tacker fixation, for small and medium-sized defects in anatomically accessible areas. However, suture fixation requires significantly longer operation time, but patients have statistically significantly less postoperative pain.

“…The authors stressed the differences in operative time: the use of sutures appears to lengthen the operating time without any real improvement in outcome. Other studies confirmed the same findings [134,135].…”

“…In a second RCT (n = 68), helical tack fixation in 2 concentric rows (titanium, double-crown technique) caused significantly more pain over a period of up to 3 months postoperatively compared to permanent transfascial sutures (polypropylene) (31). A more recent RCT evaluated another variant of fixation with regard to postoperative pain (VAS): (titanium) helical tack in double-crown technique versus a combination of (titanium) helical tacks and transfascial sutures.…”

“…A study conducted by Fortelny et al [11] with 89 patients having 110 hernia with 1-year follow-up, average VAS score was 0.4 for those whose mesh was fixed with fibrin glue and 0.9 for those in whom it was done with tacker. The study did not show any significant increase in pain caused by tacker.…”

“…The time difference was attributed to need for sutures for peritoneal flap closure in the fibrin glue group. Bansal et al [11] have compared the operating time in mesh fixation with tacker (52.6 min) and suturing (77.5 min). Tolver et al [15] conducted a study in which TAPP was done for 100 patient using fibrin glue for 50 and tack for rest of the 50 patients.…”
