A prospective, randomized, comparative clinical study of resin composite and glass ionomer cement for retrograde root filling

Abstract: Dentin-bonded RP applied onto the entire, slightly concave resection surface is a predictable apical sealant characterized by a high success rate. In contrast, retrograde root filling with CS results in an unacceptably high failure rate due to insufficient bonding strength to the concave resection surface.

“…The result of this treatment should be a reduction in bacterial numbers in the region of the neoapex to avoid inflammatory relapse followed by tooth loss. According to the literature, the success rate of this kind of surgical treatment including retrograde root filling is between 60 and 85% for a tooth survival time of more than 5 years (von Axt and Kurt, 1999;Testori et al, 1999;Jensen et al, 2002;Wesson and Gale, 2003).…”

“…Moreover, medical grade titanium has the same biological potential as mineral aggregates and is used more frequently in dental implantology today with a current success rate of more than 95% for titanium implants. In contrast, the retrograde treatment of the root apex using conventional materials has a success rate of about 70% (Jensen et al, 2002). There is still a risk of tooth loss, even when the best possible treatment is used including ultrasonic preparation under the microscope and filling with new sealers.…”

“…It should be noted that scar tissue (at 1 year) only develops in <10% of cases. [8][9][10] In contrast, some of the included studies reported an "incomplete healing" as high as 27-50%, raising the possibility of misclassification of "uncertain healing" or "failure" cases as incomplete healing. Such misclassification would result in an overestimation of the outcome in individual studies, as well as the pooled estimate.…”

“…It should be noted that scar tissue (at one year), when adequately diagnosed, has been reported to occur in <10% of teeth treated by apical surgery. [8][9][10] There was no attempt to critically investigate the percentage of the 'incomplete healing' in the individual studies that may raise the possibility of misclassification bias.…”
