A posterior approach to the elbow joint

Abstract: We describe a posterior approach to the elbow which combines the advantages of both splitting and reflecting the triceps. It gives protection to the ulnar nerve and its blood supply during the operation while providing excellent exposure of the distal humerus. During closure, the triceps muscle can be tensioned, thereby improving stability of the elbow. This approach has particular relevance to unlinked total elbow arthroplasty allowing early rehabilitation of the joint. [Br] 1999;81-B:1020-2. J Bone Joint Su… Show more

“…At the second-stage procedure the previous incision was used and a posterior approach made to the elbow. 7 The ulnar and radial nerves were identified and protected. The antibiotic-impregnated cement rods and beads were removed and the joint cavity further debrided.…”

“…At the end of the procedure the extensor mechanism was repaired to the olecranon/allograft using non-absorbable stitches. 7 Reconstructions were performed using one Stanmore (Biomet UK Ltd, Swindon, United Kingdom), one Souter-Strathclyde (Stryker Howmedica Osteonic, Limerick, Ireland) and nine Coonrad-Morrey (Zimmer, Warsaw, Indiana) replacements.…”

“…This did not describe mobilising the triceps medially, nor describe the approach to the distal humerus as has been suggested by some authors. 1,6,7 To the best of our knowledge, this is the first description of the posteriolateral approach to the distal humerus using this approach. We feel it will be of use to surgeons treating fractures of the distal humerus.…”

“…Other authors have modified this approach where the triceps is split leaving 25% medially with the ulna nerve and mobilising the remaining 75% laterally. 7 This approach appears to reduce the risk of ulna nerve injury with one temporary ulna nerve neuropraxia in a series of 106 patients. This approach still has the disadvantages of dividing and disrupting the extensor mechanism.…”

“…Shahane and Stanley [20] reported on a modification of this approach in an attempt to reduce the incidence of ulnar neuropathy. After decompression of the ulnar nerve the triceps is split leaving a quarter of triceps medially and the rest is reflected medially as a single unit.…”
