A polyclonal CD4+ and CD8+ lymphocytosis in a patient doubly infected with HTLV‐I and HIV‐1: A clinical and molecular analysis

Abstract: HTLV-I is associated with adult T-cell leukemia/lymphoma (ATL) characterized by monoclonal expansions of CD4+ T-lymphocytes. In this report we describe a histologically benign, polyclonal HTLV-I infection in a patient exhibiting both an absolute CD4+ and CD8+ lymphocytosis. Three T-cell lines containing integrated HTLV-I proviral copies established from this patient were initially polyclonal, but with time all grew out the same two clones as determined by analysis of their T-cell antigen receptor beta chain ge… Show more

“…Fifteen samples were from patients infected with HTLV-I and 17 samples were from patients infected with HTLV-II. The samples from both HTLV groups had a higher target-copy number than the samples from patients infected with HIV-1, because their blood is characterized by monoclonal expansion of HTLV-infected lymphocytes (36,37). We also included 10 samples from healthy blood donors to serve as negative controls.…”

“…The major site of HTLV-I replication in vivo appears to be CD4+ T cells (29) although less commonly B cells (30) and some CD8+ T cells (31) can be infected. Infected macrophages have not been identified in vivo.…”

“…There was no evidence for HTLV-1/11 infection, which has been linked to lymphoproliferation in the setting of HIV infection [8] in four patients tested by ELISA and polymerase chain reaction (PCR) [9].…”

“…The patient was treated with hydroxyurea on an outpatient basis and did well until the summer of 1990. She discontinued hydroxyurea and then was seen for abdominal pain on October 8,1990. The spleen was tender and enlarged and there was a tender pelvic mass.…”
