A pilot study for screening blood donors in Taiwan by nucleic acid amplification technology: detecting occult hepatitis B virus infections and closing the serologic window period for hepatitis C virus

Abstract: The use of NAT detected occult HBV and reduced HCV window period. The yield rate, especially occult HBV, was 10- to 100-fold higher than that in developed, HBV nonendemic countries. Therefore, NAT implementation for routine donor screening in a more cost-effective manner should contribute to safer blood transfusion in Taiwan.

“…In these cases, the viral load was low (Ͻ10 4 viral copies/ml) (9). In Taiwan, a study that used HBV NAT yielded 12 cases among 10,727 seronegative donations (0.11%) (18). The rate reported in this paper is 34 of 19,360 HBsAg-negative donations, or 0.18%.…”

“…It is not common in general population even in HBV endemic areas. A recent study from Taiwan suggested that the prevalence of occult HBV infection was about 0.11% among healthy blood donors with a clean, automatic, and most sensitive nucleic acid amplification assay [46]. In contrast, occult HBV infection has been more frequently identified in patients with chronic HCV infection.…”

“…This finding confirmed the significance of OBI risk in hyperendemic areas and a large proportion of anti-HBc deferrals among noninfectious donors, reinforcing the advantages of HBV NAT in such areas. HBV NAT yield was estimated at 0.11% or 1 : 909 and was evaluated as cost-effective in endemic areas [52]. A lookback study based on repository samples found 1.1% of them with OBI but none of the blood recipients with HBsAg seroconversion or clinical symptoms of posttransfusion HBV [53].…”
