This study was performed to determine the risk factors associated with systemic complications of maxillofacial space infection (MSI), and to propose an objective evaluation index - severity score of MSI. 457 MSI patients from Jan 2010 to Dec 2020 were reviewed retrospectively. The predictor variables included demographic, origin of infection, underlying systemic disease, pre-hospital medication history, laboratory examinations and severity scores of space infection. The severity score of space infection was proposed to evaluate the airway compromise of anatomic spaces. The primary outcome variable was the complication. The impact factors of complications were analyzed using univariate analysis and multivariate logistic regression. 457 patients were included (average age 46.3 y, male to female ratio 1.43:1). Among them, 39 patients developed postoperative complications. In the complication group, there were 18 patients (46.2%) with pulmonary infection, and two patients died. We found that the history of diabetes mellitus (OR=4.74, 95% confidence interval (CI)=2.22, 10.12), high temperature (≥39°C) (OR=4.16, 95% CI=1.43, 12.06), advanced age (≥65 y) (OR=2.88, 95% CI=1.37, 6.01), and severity score of space infection (OR=1.14, 95% CI=1.04, 1.25) were independent risk factors for complications of MSI. All the risk factors needed to be closely monitored. Severity score of MSI was an objective evaluation index to predict complications.

Objectives Aim of this study was to investigate clinical characteristics and individualized treatment experience of different types of deep neck space infections(DNSI) . Design Retrospective study. Setting Tertiary Referral Otolaryngology Centre. Participants 51 subjects with deep neck space infections. Main outcome measures In 51 subjects with DNSI, 15 cases were not treated with abscess incision and drainage; 36 cases were treated by incision, drainage and dressing change: 23 cases were treated with traditional incision and drainage and dressing change; 13 cases were given the combined treatment of incision and drainage, pyogenic cavity aerobic therapy and negative pressure drainage. The average number of days of dressing change and average hospital stay were compared between the two groups Results The average hospitalization days in the traditional dressing group were (26.74±3.393) days, and the average days of postoperative dressing change were (25.91±3.427) days. The average hospitalization days in pyogenic cavity aerobic therapy + negative pressure drainage were (11.08±2.108) days, and the average days of postoperative dressing change were (3.692±0.208) days. All 51 patients were cured. Conclusion Individualized treatment can minimize the trauma of patients according to each patient’s own situation. The pyogenic cavity aerobic therapy has proved to be a good method for dressing change after deep neck space infections, and the satisfactory curative effect.

Deep neck spaces are regions of loose connective tissue present between three layers of deep cervical fascia, namely, superficial, middle, and deep layers. The investing layer is the superficial layer, the pre-tracheal layer is the intermediate layer, and the prevertebral layer is the deep layer. Deep neck space infection (DNI) is defined as an infection in the potential spaces and actual fascial planes of the neck. Spread of infection occurs along communicating fascial boundaries. These deep neck spaces may be further classified into 3 anatomic groups, relative to the hyoid bone: Those located above the level of the hyoid, those that involve the entire length of the neck, those located below the level of hyoid. The patterns of infection may include abscess formation, cellulitis, and necrotizing fasciitis. Antibiotics and surgical drainage form the mainstay of treatment. There are some spaces in the neck present between these layers of deep cervical fascia. These deep neck spaces are filled with loose connective tissue. Deep neck space infection involves the spaces and fascial planes of the neck. Spread of infection occurs along communicating fascial boundaries after overcoming the natural resistance of the fascial planes. With relation to the hyoid bone, these deep neck spaces are further classified as follows: 1. Spaces above the level of the hyoid bone (peritonsillar, submandibular, parapharyngeal, masticator, buccal, and parotid spaces). 2. Spaces that involve the entire length of the neck (retropharyngeal, prevertebral, and carotid spaces). 3. Spaces located below the level of hyoid bone (anterior visceral or pre - tracheal space). Infection may present either as abscess, cellulitis, or necrotizing fasciitis. The mainstay of the management are antibiotics and surgical drainage.
