Introduction. Trigeminal neuropathy is most often secondary to trauma. The present study explores the underlying causes and the factors that influence recovery. Material and methods. A retrospective case study was made involving 63 patients with trigeminal neuropathy of traumatologic origin, subjected to follow-up for at least 12 months. Results. Fifty-four percent of all cases were diagnosed after mandibular third molar surgery. In 37 and 19 patients the sensory defect was located in the territory innervated by the mental and lingual nerve, respectively. Pain was reported in 57% of the cases, and particularly among the older patients. Regarding patient disability, quality of life was not affected in three cases, while mild alterations were recorded in 25 subjects and severe alterations in 8. Partial or complete recovery was observed in 25 cases after 6 months, and in 32 after one year. There were few recoveries after this period of time. Recovery proved faster in the youngest patients, who moreover were the individuals with the least pain. Conclusion. Our patients with trigeminal neuropathy recovered particularly in the first 6 months and up to one year after injury. The older patients more often suffered pain associated to the sensory defect. On the other hand, their discomfort was more intense, and the patients with most pain and the poorest clinical scores also showed a comparatively poorer course. Changes in Morphology of Alveolar Buccal Walls Following Atraumatic Internal Root Fragmentation

SUMMARY:The buccal alveolar wall represents the most important structure to provide shape and volume of the alveolous following tooth extraction. The aim of the study was the evaluation of buccal alveolar bone structures following minimally invasive surgery. In 15 patients (3 male, 12 female), aged 20-67 years, 3 central incisors, 5 lateral incisors, and 7 bicuspids were removed using flapless enucleation. The enucleation comprised endoscopically assisted mesiodistal root sectioning with inward fragmentation of the oral and apical parts followed by internal reduction of the buccal root lamella. Buccal bone height before extraction was 10.61 mm, following extraction 10.50 mm. Crestal width of the buccal bone plate was 1.11 mm before and 1.40 mm after tooth removal. Apical buccal bone width before was 0.66 mm and after extraction 0.40 mm. Gingival height was 13.58 mm before and 13.56 mm following extraction. Following transalveolar enucleation, the buccal alveolar bone wall remains unchanged concerning height and crestal width.

A jelen tudományos közleményt a szerzők a Pécsi Tudományegyetem alapításának650. évfordulója tiszteletére közlik.Az alsó bölcsességfogak coronectomiája a bölcsességfog részleges eltávolításával hivatott az esetleges nervus alveolaris inferior sérüléseit kiküszöbölni. Az eljárásnak van néhány sarkalatos pontja, úgymint a fog pulpalis, periapicalis preoperatív állapota, a koronai sectio és koronaeltávolítás, illetve a sebzárás mikéntje, valamint a páciens posztoperatív követése és az esetlegesen felmerülő szövődmények ismerete és kezelése. Szakirodalmi adatok szerint a coronectomia kevesebb szövődménnyel bír, mint a hagyományos, teljes fogeltávolítások, és a nervus alveolaris inferior sérülések szempontjából a coronectomia egyértelműen és kifejezetten a biztonságosabb eljárás. A szerzők célja volt a szakirodalmat áttekintve, az eljárás indikációiról és kontraindikációiról, korrekt kivitelezéséről és az esetleges sikertelenségé-nek okairól, gyakoriságairól átfogó képet adni, előmozdítva az eljárás hazai népszerűsítését. The coronectomy procedure has several crucial points, such as the pulpal, periapical preoperative conditions of the tooth, the way of crown sectioning and decoronation, the wound closure and the length of patients' follow up. In addition it is important to identify and manage possible intraoperative or postoperative complications correctly. According to the literature, the complication rate of coronectomy is usually lower, than that of total teeth removal, furthermore considering inferior alveolar nerve injuries, coronectomy is significantly the safer procedure.
