Background: Stroke is the fourth leading cause of death in Europe, represents one of the most common causes of disability in adult patients, and involves considerable short- and long-term social and healthcare costs. The effectiveness of deep dry needling (DDN) on affected arm functionality was assessed throughout 8 weeks of treatment in patients with stroke in the subacute phase. Methods: Eighty patients were included in this two-group non-randomised study after a propensity score analysis was carried out. Both groups received standard physiotherapy treatment on the affected arm. The needling group also received six sessions of DDN during the 8-week period. Patients were evaluated before and after each session using the Fugl-Meyer upper extremity (FM UE) scale, the modified modified Ashworth scale (MMAS), the resistance to passive movement scale (REPAS) and a 10-point numeric pain rating scale (NPRS 10). The Brunnstrom recovery stage was recorded at the beginning and at the end of the study, and the EuroQoL quality of life survey was completed at the beginning of the study, after the first month of treatment and at the end of the study. Results: Patients treated with DDN showed a reduction in spasticity measured using the REPAS (p < 0.001) and the MMAS (p < 0.05). There was also an improvement in the Brunnstrom recovery stages (p < 0.05). Conclusion: The addition of a specific DDN treatment to a standard physiotherapy treatment appeared to lead to a higher reduction in spasticity in the affected arm; however, it did not provide additional changes in functionality, pain and quality of life. Further studies with a randomised controlled trial design are required to confirm our findings.

Commonly used sensors like accelerometers, gyroscopes, surface electromyography sensors, etc., which provide a convenient and practical solution for human activity recognition (HAR), have gained extensive attention. However, which kind of sensor can provide adequate information in achieving a satisfactory performance, or whether the position of a single sensor would play a significant effect on the performance in HAR are sparsely studied. In this paper, a comparative study to fully investigate the performance of the aforementioned sensors for classifying four activities (walking, tooth brushing, face washing, drinking) is explored. Sensors are spatially distributed over the human body, and subjects are categorized into three groups (able-bodied people, stroke survivors, and the union of both). Performances of using accelerometer, gyroscope, sEMG, and their combination in each group are evaluated by adopting the Support Vector Machine classifier with the Leave-One-Subject-Out Cross-Validation technique, and the optimal sensor position for each kind of sensor is presented based on the accuracy. Experimental results show that using the accelerometer could obtain the best performance in each group. The highest accuracy of HAR involving stroke survivors was 95.84 ± 1.75% (mean ± standard error), achieved by the accelerometer attached to the extensor carpi ulnaris. Furthermore, taking the practical application of HAR into consideration, a novel approach to distinguish various activities of stroke survivors based on a pre-trained HAR model built on healthy subjects is proposed, the highest accuracy of which is 77.89 ± 4.81% (mean ± standard error) with the accelerometer attached to the extensor carpi ulnaris.

Most post-stroke patients experience varying degrees of impairment in upper limb function and fine motor skills. Occupational therapy (OT) with other rehabilitation trainings is beneficial in improving the strength and dexterity of the impaired upper limb. An accurate upper limb assessment should be conducted before prescribing upper limb OT programs. In this paper, we present a novel multisensor method for the assessment of upper limb movements that uses kinematics and physiological sensors to capture the movement of the limbs and the surface electromyogram (sEMG). These sensors are Kinect, inertial measurement unit (IMU), Xsens, and sEMG. The key assessment features of the proposed model are as follows: (1) classification of OT exercises into four classes, (2) evaluation of the quality and completion of the OT exercises, and (3) evaluation of the relationship between upper limb mobility and muscle strength in patients. According to experimental results, the overall accuracy for OT-based motion classification is 82.2%. In addition, the fusing of Kinect and Xsens data reveals that muscle strength is highly correlated with the data with a correlation coefficient (CC) of 0.88. As a result of this research, occupational therapy specialists will be able to provide early support discharge, which could alleviate the problem of the great stress that the healthcare system is experiencing today.
