Objectives To evaluate the effects of photobiomodulation (PBM) in gingival lesions resulting from autoimmune diseases; to compare PBM and topical corticosteroid (CS) treatment; and to assess PBM outcome over time of follow-up. Materials and methods A comprehensive electronic search was performed in four electronic databases. Treatment effects were measured through visual analog scale of pain (VAS) and clinical evolution of lesion (Thongprasom scale for oral lichen planus (OLP)). Meta-analysis was performed to compare PBM with topical corticosteroid treatment and to evaluate PBM effect over time of follow-up. Results Seventeen studies were included in this review, of which six were used for the meta-analysis. Meta-analysis results showed no significant differences between PBM and topical CS in pain reduction at baseline (MD = 0.20, 95% CI = − 0.92, 1.32, p = 0.72) and 60-day follow-up (MD = 0.63, 95% CI = − 3.93, 5.19, p = 0.79); however, VAS showed significant pain reduction when compared before and after PBM at 30-day (MD = − 3.52, 95% CI = − 5.40, − 1.64, p = 0.0002) and 60-day (MD = − 5.04, 95% CI = − 5.86, − 4.22, p < 0.00001) follow-up. Thongprasom clinical scale for OLP also showed significant improvement at 30-day follow-up (MD = − 2.50, 95% CI = − 2.92, − 2.08, p < 0.00001) after PBM. Conclusion PBM led to significant reduction of pain and clinical scores of the lesions, not having shown significant differences when compared to topical CS. Clinical relevance PBM has been used in the treatment of autoimmune gingival lesions, but so far there is little strong evidence to support its use.

Background: Oral lichen planus (OLP) is an intractable chronic inflammatory disease associated with dyskeratosis. Several clinical classifications of OLP have been reported. However, only a few detailed studies have investigated the clinical symptoms, classifications, and treatment effect. In this study, we investigated the clinical features of OLP at the time of the initial diagnosis and classified it using the clinical visual examination type classification. Furthermore, we evaluated whether there is a difference in the clinical characteristics and effectiveness of symptomatic treatment between the typical reticular type and other types of OLP.Methods: A total of 132 patients with OLP were included. Patients’ age, sex, chief complaints, clinical morphological forms, lesion sites, cytological and histopathological findings, therapeutic strategies, and therapeutic responsiveness were evaluated. Results: Of the 132 patients with OLP, there were 42 men and 90 women, with a mean age of 66.4±13.0 years. The buccal mucosa was the most affected site (n=95). The reticular form was the most common clinical morphological form, observed in 124 cases. Biopsy was performed in 48 patients. The erosive form was the most common among the biopsied lesions. Regarding therapeutic strategies, topical steroids were prescribed to 85 patients. Among these patients, 47 experienced symptomatic relief and 38 showed no changes in symptoms. Based on the clinical morphological form, the rates of symptomatic relief following topical steroid use were 70.4% for the reticular form and 15.9% for the erosive form. In patients with erosive and plaque-like forms, it was difficult to achieve symptomatic relief with topical steroid use, whereas in those with other forms, a moderate response was observed. Conclusion: Topical steroids are often ineffective in patients with the erosive or mixed form, and these clinical morphological forms are associated with a higher risk of malignant transformation compared with the other forms. Therefore, early histological examination is important.Trial registration number: Not applicable.

Disease severity scoring systems in mucosal lichen planus: A systematic review

Several scoring systems have been developed to evaluate disease severity in mucosal lichen planus, but only a few have been validated to ensure reproducible and accurate assessment of disease severity. The current systematic review was undertaken to identify clinical severity scoring systems in mucosal lichen planus that have undergone validity or reliability testing and to describe their operating characteristics. We performed a bibliographic search in five databases from their inception to October 2022 for severity scoring systems in mucosal lichen planus that have undergone validity or reliability tests. Quality assessment was conducted using the Joanna Briggs Institute Critical Appraisal tools. We have included 118 studies and identified 11 clinical severity scoring systems for oral lichen planus that have undergone validity or reliability testing. Of these, the most reported were the Thongprasom score, the Oral Disease Severity Score (ODSS) and the REU (Reticular/hyperkeratotic, Erosive/erythematous, Ulcerative) scoring systems. We did not identify clinical scoring systems for extraoral mucosal lichen planus that have undergone validity or reliability testing. The ODSS and REU scoring systems have undergone the highest number of validation attempts and reliability assessments for oral lichen planus respectively. However, we have identified numerous factors that have hampered the universal adoption of a standardised scoring system. There is a need for the development and validation of scoring systems for extraoral mucosal lichen planus.
