A new percutaneous approach for the treatment of hydatid cysts of the liver

Abstract: Long term results indicate that this new percutaneous treatment modality of the hydatid disease of the liver is an effective and safe method without causing major complications. Percutaneous treatment of hydatid cysts of the liver offers good results and should be the first choice, especially for patients who are contraindicated to surgery.

“…As polidocanol has been proposed for use as a sclerosing compound in treatment of diverse hepatic diseases such as intrahepatic cysts (Kaneko et al 2002;Ormeci et al 2001), the present data demonstrating a potential deleterious effect if polidocanol enters the hepatic vascular system have to be taken into account and polidocanol instillation may be performed under great caution.…”

“…As polidocanol is routinely used for sclerosing varices and other vascular pathologies in the gastrointestinal tract (Morales and Baum 2003;Vlavianos and Westaby 2001) and it has recently been proposed for use in the treatment different hepatic diseases (Kaneko et al 2002;Ormeci et al 2001), the present study addressed its effects when administered to the hepatic vascular system because the liver plays an important role in the metabolism of exogenous substances due to its anatomical position between the gastrointestinal tract and the systemic circulation.…”

“…Despite these results and some progress in medical treatment, surgery is still the treatment of choice for uncomplicated hydatid disease of the liver [48,76,103]. Additional alternative methods comprise percutaneous evacuation (PEVAC) and radiofrequency thermal ablation [104,105]. However, only initial experience has been gained in these options.…”

“…Pure ethanol (95% concentrate), 20% hypertonic saline, Betadine, silver nitrate, 50% dextrose, 20 % mannitol, 10% albendazole solution are all used for this purpose (12,13). Documented use of 1% polydocanol was first used in this treatment method (14). The significant advantage of 1% polydocanol is the closing of the connection between hydatid cysts and blood vessels or biliary tracts, if there are any.…”
