A New Modification of Fan Flap for Large Lower Lip Defects

Abstract: We believe that the unilateral extended fan flap is a reliable and safe option with satisfactory functional and aesthetic results for total and near-total (more than 80%) lower lip defects.

“…The lip is a complex, multilayered anatomical structure that participates in several basic human functions, such as speech, mastication, drinking, and oral competence 2,5,7 . Limited defects can be reconstructed with a rearrangement of the surrounding tissue using transposition flaps (lip switch, 20 nasolabial, 21,22 bilobed platysma flap 23 ), advancement/rotation flaps (Yu, 24 Bernard-Webster, 25 Karapandzic, 26 Gillies 27 ) or a combination of flaps 28–31 . These local flaps produce the best functional and aesthetic results because they replace “like-with-like” tissue.…”

“…9,20,21 They are integrated by 3 layers: mucosa, muscle, and skin. 4,9,12,15 The musculature of the lips is a complex structure, and it is made up by the orbicularis oris muscle plus a dozen paired muscles with lift, depression, and sphincter functions. The modiolus is a fibrous structure anchored to the deep layers of the skin in the corner of the mouth, serving as an insert region for some perioral muscles.…”

“…Although numerous flap designs and modifications have been described, only a few flap concepts and techniques provide reliable and consistent results. [8][9][10][11][12][13][14][15] Transoral crosslip flaps such as the Abbé 9 and Estlander 10 flaps are occasionally suitable for broader lower lip defects. Advantages of these flaps include the transfer of similar tissue and the frequent preservation of lip function.…”
