SummaryWe evaluated an alternative technique for ultrasound-guided proximal level obturator nerve block that might facilitate needle visualisation using in-plane ultrasound guidance. Twenty patients undergoing transurethral bladder tumour resection requiring an obturator nerve block were enrolled into a prospective observational study. With the patient in the lithotomy position, the transducer was placed on the medial thigh along the extended line of the inguinal crease, and aimed cephalad to view a thick fascia between the pectineus and obturator externus muscles that contains the obturator nerve. A stimulating nerve block needle was inserted at the pubic region and advanced inplane with the transducer in an anterior-to-posterior direction. Eight ml levobupivacaine 0.75% was injected within the fascia. The median (IQR [range]) duration for ultrasound identification of the target and injection were 8.5 (7-12 [5-24]) s and 62 (44.5-78.25 [39-383]) s, respectively. All blocks were successful. A cadaver evaluation demonstrated that the dye injected into the target fascia using our technique travelled retrogradely through the obturator canal, and surrounded the anterior and posterior branches of the obturator nerve both proximally and distally to the obturator canal. We believe that this is a promising new technique for ultrasound-guided proximal level obturator nerve block.

ÖzetAmaç: Obturator sinir bloğu, transüretral rezeksiyon sırasında obturator refleksin gelişmesini önlemek için tanımlanmıştır. Tanımlanmasından bu zamana kadar birçok farklı yaklaşım geliştirilmiştir. Bu çalışmada, interadduktor yaklaşımının sonuçlarının görülmesi hedeflenmiştir. Gereç ve Yöntem: Etik kurul onayı alındıktan sonra; Ekim 2013-Ekim 2014 tarihleri arasında transüretral rezeksiyon cerrahisi geçirmiş olan hastaların dosyaları geriye dönük olarak tarandı. Bulgular: Transüretral rezeksiyon geçiren 137 hasta olduğu bulundu. Cerrahi anestezi amacıyla spinal anestezi ve obturator sinir bloğu kombinasyonu uygulanan hasta sayısı 69'du (2'si kadın, 67'si erkek). Bütün obturator sinir blokları interadduktor yaklaşımla, ultrason eşliğinde yapıldı. Obturator refleksin görülmesinden dolayı iki hastada bloğun başarısız olduğu saptandı. Bütün hastaların operasyonları herhangi bir komplikasyona rastlanmadan tamamlandı. Sonuç: Obturator sinir bloğu obturator refleksin önlenmesi için etkili bir bloktur. Obturator sinir bloğunun spinal anestezi ile kombine edilmesi transüretral cerrahilerin anestezisinde güvenli bir yöntem olarak görülmektedir. Obturator sinir bloğunda ultrason rehberliği blok başarı oranının artmasına yardım etmekte ve hasta güvenliği açısından ek avantajlar sağlamaktadır. SummaryObjectives: Obturator nerve block is recommended to prevent obturator nerve reflex during transurethral resection. However, alternate techniques have been developed. The aim of the present study was to examine outcomes of interadductor approach. Methods: After obtaining approval from the ethics committee, files of patients who underwent transurethral resection surgery between October 2013 and October 2014 were reviewed. Results: A total of 137 transurethral resection patients were identified, in 69 ( 2 women, 67 men) of whom a combination of spinal anesthesia and obturator nerve block was used. Obturator nerve blocks were ultrasound-guided with interadductor approach. Nerve block was unsuccessful in 2 cases due to obturator nerve reflex. Surgeries were performed without complication. Conclusion: Obturator nerve block is an effective method of preventing obturator nerve reflex. Combination of obturator nerve block and spinal anesthesia seems to be a safe method of anesthesia in transurethral surgery. Ultrasound guidance improves success rates and provides additional advantages for patient safety.

ÖZETAmaç: Bu çalışmada mesane(M) yan duvar tümörlerinin transüretral rezeksiyonunda (TUR-M) obturator sinir bloğunun aduktor kasılmalar ve buna bağlı oluşabilecek komplikasyonları engellemedeki etkinliği araştırıldı Gereç ve Yöntem: Hastanemizde 2010 yılı Nisan ve Aralık ayları arasında TUR-M operasyonu geçiren toplam 158 hasta gözden geçirilmiştir. Bu hastalardan preoperatif dönemde yapılan US raporlarına göre tümör yerleşimi yan duvar olarak belirlenen 82 hasta değerlendirildi. Bunlardan sadece spinal anestezi yapılan 40 hasta (Grup I) ve spinal anesteziye ek olarak obturator blok yapılan 32 hasta (Grup II) aduktor kas kontraksiyonu, mesane perforasyonu ve genel anesteziye geçiş oranları açısından geriye dönük değerlendirildi. Bulgular:Her iki gruptaki hastaların yaş, cinsiyet, boy, kilo, ASA riski, cerrahi süreleri karşılaştırıldığında istatiksel olarak anlamlı fark bulunmadı (p>0,05). Grup I'de aduktor kas kontraksiyonu (p=0,001) ve mesane perforasyonu (p=0,012) görül-me sıklığı Grup II'den anlamalı olarak yüksek bulundu, genel anesteziye geçiş oranında ise gruplar arasında anlamlı fark bulunmadı (p=0,124). Sonuç:Yan duvar yerleşimli mesane tümörlerinin TUR-M operasyonlarında spinal anesteziye eklenen obturator sinir bloğu, oluşabilecek aduktor kas kontraksiyonu ve buna bağlı olarak komplikasyonları önlemede etkili bir yöntemdir.Anahtar Sözcükler: Aduktor kas kontraksiyonu, mesane delinmesi, mesane tümörü, obturator sinir bloğu transüretral rezeksiyon, TUR komplikasyonu

Abstract: The new inguinal approach decreases patient discomfort and pain of block placement as well as the time and sedation and analgesics required for a similar quality of sensory and motor block compared with the pubic tubercle approach.
