Endometriosis affects up to 15% of reproductive-aged women. Consideration of this nosological entity in a new aspect is impelled by the evidence that 0.5-1% of cases are complicated by endometriosis-associated ovarian neoplasms represented mainly by the endometrioid and clear-cell subtypes of tumors. This category also includes seromucinous ovarian tumors (SMOTs). Objective. To investigate the morphological and immunohistochemical (IHC) features of SMOTs in reproductiveaged women, including pregnant ones. Subjects and methods. Ovarian tumor samples from 15 patients who had been operated on in 2012-2016 were analyzed comprehensively at the National Medical Research Center of Obstetrics, Gynecology, and Perinatology: IHC examination was conducted using the biomarkers of estrogen and progesterone receptors and Ki-67. Results. Borderline SMOTs were characterized by papillary growths on the inner surface (in one case) and on the outer surface, with an adhesive process with the serous surface of the corpus uteri (FIGO Stage 1c), and mixed epithelial structures; leukocyte infiltration was observed in four cases. The epithelium of seromucinous carcinoma (n = 1) was similar to that of serous, endocervical, and endometrial types at the same time. All the samples exhibited low Ki-67 expression and high positive receptor immunoreactivity. Endometriosis diagnosed in nine women was accompanied by benign seromucinous cystadenomas (n = 4), borderline tumors (n = 4), and carcinoma (n = 1). Conclusion. Most of the examined SMOTs showed the areas, covered with the endometrioid epithelium, with adjacent endometrial stroma, which indicated the development of carcinoma in the presence of endometrioid cyst. The revealed facts suggest that endometriosis plays an etiological role in developing SMOTs. When detected, most neoplasms are localized (limited to ovarian tissue, FIGO stage 1a) and characterized by a favorable prognosis as a whole.

Цель исследования: определение особенностей ультразвуковой визуализации пограничных опухолей яичников и обзор литературы по соответствующей тематике. Материал и методы исследования: обследована 21 пациентка с 22 пограничными опухолями яичников. Медиана возраста пациенток составила 48 лет, минимальное - максимальное значения - 25-58 лет. При гистологическом послеоперационном исследовании у 11 пациенток оказались односторонние серозные пограничные опухоли яичников, у 9 - односторонние муцинозные пограничные опухоли яичников, у 1 - двусторонние пограничные опухоли яичников - справа муцинозная и слева сероз ная. Все опухоли (n = 22) были разделены на две группы: 1-ю группу составили 12 серозных пограничных опухолей яичников, 2-ю группу - 10 муцинозных пограничных опухолей яичников. Результаты исследования: для пограничных опухолей яичников не типичны однокамерный (без включений) и солидный типы эхоструктуры. Для серозных пограничных опухолей яичников более характерен однокамерный солидный тип эхоструктуры (P 0,05 при сравнении с муцинозными пограничными опухолями яичников). Для муцинозных пограничных опухолей яичников более характерно многокамерное ультразвуковое строение (с солидными включениями или без них) (P 0,05 при сравнении с серозными пограничными опухолями яичников). Содержимое камер муцинозных пограничных опухолей яичников в 4 из 10 (40%) случаев имело взвесь высокой эхогенности, которую трудно дифференцировать с солидным компонентом. В некоторых муцинозных пограничных опухолях яичников встречается сотовидный (1 случай) или многокамерный (3 случая) тип строения солидных включений (эти типы не встречаются в серозных пограничных опухолях яичников). Для пограничных опухолей яичников не характерен асцит. В большинстве (21 из 22 (95%)) пограничных опухолей яичников определяется скудный (2-й тип по терминологии IOTA) или умеренно выраженный (3-й тип по терминологии IOTA) кровоток (различия между серозными и муцинозными опухолями недостоверны). Медиана Vmax внутриопухолевого артериального кровотока в пограничных опухолях яичников составила 10,2 см/с, минимальное - максимальное значения - 4,2-15,2 см/с, RI - 0,48, 0,31-0,55 соответственно. Заключение: при проспективной оценке правильно диагностировать пограничные опухоли яичников при ультразвуковом исследовании удалось в 14 из 22 (64%) случаев, из них серозные пограничные опухоли яичников - в 6 из 12 (50%) случаев, муцинозные - в 8 из 10 (80%) случаев. При проспективной оценке правильно диагностировать недоброкачественный характер опухолей яичников удалось в 20 из 22 (91%) случаев, из них в 11 из 12 (92%) случаев серозных пограничных опухолей яичников, в 9 из 10 (90%) случаев муцинозных пограничных опухолей яичников. Background The 2014 WHO Classification of ovarian neoplasms introduced a new entity of seromucinous tumors associated with endometriosis. These tumors encompassed a spectrum from benign to malignant and included seromucinous cystadenoma/ cystadenofibroma, seromucinous borderline tumor/atypical proliferative seromucinous tumor and seromucinous carcinoma. However, the 2020 WHO Classification of Female Genital Tumours removed seromucinous carcinomas as a distinct entity and recategorized them as Endometrioid carcinomas with mucinous differentiation. Here we describe clinico-morphologic features of seromucinous tumors recategorizing cases originally diagnosed as seromucinous carcinoma in light of 2020 WHO classification and present detailed review of literature. Methods Slides of seromucinous tumors were reviewed. Special emphasis was given to evaluation of stromal invasion. Follow-up was obtained. Results Ten cases were diagnosed. Mean age was 40 years. Four cases were bilateral. Mean size was 19 cm. Grossly; luminal papillary projections were seen in 6 cases. Tumors demonstrated a papillary architecture with papillae lined by stratified seromucinous epithelium showing nuclear atypia. Stromal invasion was seen in 4 cases. Six cases were reported as borderline seromucinous tumors and 4 cases originally diagnosed as seromucinous carcinoma were recategorized as endometrioid carcinoma with mucinous differentiation on review. Endometriosis was seen in 4 cases. CK7, PAX8 and ER were positive in 7/7 cases. Two cases showed extra-ovarian involvement. Follow up was available in 7 cases. Six patients were alive and well at follow up ranging from 8 to 46 months. Six patients received chemotherapy postoperatively. One patient with carcinoma died of disease 18 months postoperatively. Conclusion In our series, 4 cases were originally diagnosed as seromucinous carcinomas. However, these were recategorized in light of the 2020 WHO Classification of Female Genital tumors as endometrioid carcinomas with mucinous differentiation. Six cases were diagnosed as seromucinous borderline tumors. Thus, majority of cases were borderline in agreement with published literature.
